You are doing a great job adjudicating all of these special circumstances. I agree, I would not have had my mace out, and I would use my two attacks rather than raising my shield. I will probably use reactive shield as my reaction then.

I would assume a 26 works this time if a 23 did last time.
FYI, as far as I know you have to make the flat check for each action that targets a concealed or hidden creature (including subordinate actions). It's not once per round.

Open the PDF in a PDF editor. I just use my browser, in this case Microsoft Edge. There should be some icons at the top, one of which (probably the one with a big T in it) may be used to add text. Select that icon, move your curser over the section where you want to add text and start typing.

Print it out and use a pen? Use the mark-up feature of acrobat? Dedicate a monitor permanently and use a sharpie on the screen? Take a screenshot and add text using Paint or the equivalent.
Some of these ideas may be better than others.

No worries, my apologies if any of my answers came off as flippant, I've been having a grumpy day, was just trying to point out there are a lot of ways to mark-up pdfs and that depend on your OS/setup etc.
Big T as usual has the easiest solution.
