Urth Stormraven
Urth casts See Invisibility and regains a first level spell slot, because he's an expert diviner.
GM Infinity
Narnel also doesn't seem to find anything particularly dangerous on the door other than the odd perforations.
Molok steamrolls into the darkness and nothing happens, but then again who would know considering he is engulfed in magical darkness?
Molok, as mentioned by Lyraath, he saw a coffin and some treasure in here with his psionic sight. Interacting with anything?
Godric Alden
”We should have kept some corpses. Throw them into a room like this to set the traps off. Or maybe a summoned monster to do the deed with more finesse.”
GM Infinity
So, any bright ideas? Lyraath mentioned what he could see inside, a treasure cache, a coffin, an upturned stone plate, wall markings.
The magical darkness if of course an annoying obstacle, if theres nothing youve got currently to deal with it, perhaps later something will come about.

GM: 1d20 + 9 ⇒ (17) + 9 = 26
Molok suddenly feels something brush up against his neck...

...and soon the stoneman is grappled nearly halfway off the ground by the throat, feeling two sharp points of daggers sink into his neck...
Dmg: 1d6 + 4 ⇒ (4) + 4 = 8 piercing
Dmg: 3d6 ⇒ (6, 6, 3) = 15 necrotic
Molok, you lose 23 hp. In addition, you lose 15 point from your maximum hp.
Molok feels quite drained, being magically drained of a large amount of blood quite rapidly. The rest of you hear some muffled sounds and gurgling noises, but its hard to tell just what the hell is happening.
G: 1d20 + 2 ⇒ (12) + 2 = 14

Another 30 total damage to Molok, with 13 being a further reduction in maximum HP. Everyone up
Round 2:
Enemy
All PCs <---UP
GM Infinity
Someone could call out a pinpointed location (using search action or Molok knows) that could allow others to target the creature with disadvantage. Dragging it into the light could also work, but Molok would have to take over the grapple. If I were Urth, I suppose I might consider picking a creature with blindsight or tremorsense to Polymorph into.
Lyraath
Lyraath reactivates truesight as a bonus action, and then manifests Psychic Hammer at it to try and pull whatever creature it is away from Molok towards us - even out of the darkness if possible, but probably not since can only pull 10 feet if successful.
Only after that, he will yell out what creature he sees and where it roughly is in the room.
Psychic Hammer: Strength Save DC 15 or take 2d6 force damage and pulled 10 feet.
Psychic Hammer damage: 2d6 ⇒ (2, 3) = 5
GM Infinity
Lyraath, you see a pale, unarmed, unarmored humanoid with sharp pointy teeth embracing Molok and sucking the blood from his neck.
Strength vs Lyr: 1d20 + 4 ⇒ (4) + 4 = 8
Fail, however the creature uses one of it Legendary Resistances to save instead. It can do this twice more, for those who haven't fought legendary creatures before
It truly startles at the psionic attack, as if it has not experienced such in a very long time, or ever at all. However, you feel the twisted nature of the creature fighting you back, cursed spirit vs planar mind, and ultimately the energy is wasted.
Now that Lyraath has called it out, other PCs could make attack rolls at disadvantage.
Round 2:

Lyraath, Molok, Godric, Urth, Narnel <---UP
Map updated
edit: Molok, your escape grapple DC is 18 here
Urth Stormraven
Thanks Lyraath
Urth moves up to the edge of the magical darkness and casts Polymorph. Expanding into the room to support Molok appears a young black dragon with blindsight.
GM Infinity
Urth, sorry but a young black dragon is a dragon, not a beast. You would need True Polymorph or Shapechange to do that form.
Heres a handy creature list, but it might be outdated: Creatures valid with Polymorph spell.

Round 2, Kensei's Shot, Deft Strike (1 Ki)
Narnel aims and fires his longbow.
LB Att: 1d20 + 7 ⇒ (19) + 7 = 26
LB Att Disadv: 1d20 + 7 ⇒ (8) + 7 = 15
LB Dam: 1d8 + 4 + 1d4 + 1d6 ⇒ (3) + 4 + (2) + (6) = 15

LB Att Disadv: 1d20 + 7 ⇒ (3) + 7 = 10
LB Dam: 1d8 + 4 + 1d4 ⇒ (2) + 4 + (1) = 7
GM Infinity
Narnel's arrows whistle into the darkness, striking what sounds like hard stone in the distance.
Molok, your speed is 0 while grappled. You could escape the grapple using an action, or you could also try shoving it twice with your action + extra attack. A third option would be to shove it to break your grappled condition, move, grapple with extra attack, continue move.
GM Infinity
Molok tries to throw the creature off him but its like pushing a solid wall. He takes a different approach and finally manages to slip free of its grasp.
Shove fail, grapple success. You broke the grapple with your second action, so its still adjacent to you but you are no longer grappled. You still have a pure move (with AoO) if desired.
Urth Stormraven
I tried unsuccessfully. Still stuck on actions. I'm missing Dispel Magic
Urth looks for anything invisible.
GM Infinity
Urth, I think you have some options like Bless, Shatter, or ranged attack but dont want to tell you what to play. Dispel would be handy here, indeed. Retreat would be wise if not for the insanely dangerous chains and geysers room that direction.
Godric gazes into the void of Blackrazor and back at the others, equally at a loss for what to do. Suddenly, you hear the creature roar a screeching, piercing roar of bloodlust. Lyraath with his truesight can see the creature calling upward into the ceiling, its eyes glazed over a solid black.
GM: 1d4 ⇒ 1
Party up again.
Round 3:

Lyraath, Molok, Godric, Urth, Narnel <---UP
Godric Alden
Im here! My apologies.
Seeing his comrade grappled, Godric decided to endulge his sword.
Hit: 1d20 + 10 ⇒ (12) + 10 = 22

Damage: 1d8 + 9 ⇒ (3) + 9 = 12
Smite: 2d8 ⇒ (5, 5) = 10
He would escape this dungeon. He would free Blackrazor. His mistress willed it. He would make it so.
Urth Stormraven
If Urth can affect Molok in the darkness with Bless, Urth will cast Bless 3rd level to affect everyone.
"Volkur! Guide us!" Urth shouts.
GM Infinity
Godric, the creature is within magical darkness. Lyraath has indicated its position, but to attack without disadvantage you would need to use your Detect Evil (Divine Sense) ability. That's an action so you would not be able to attack the same round.
So, you could A: roll for disadvantage. B: Detect Evil then attack next round. Or I guess C: GM could be nice and say the turn you skipped earlier was you activating the sense. Do you spend the usage?
At Urth's invocation a swelling of energy bursts from the hearts of all, like a ship at sea under a tide swell into the warm sunlight. PCs get d4 on attacks or saves as long as Urth is concentrating
Godric Alden
As much as I’d like to use it, I’ll go with disadvantage. Think it is better than losing a turn.
Attack 22: 1d20 + 10 ⇒ (13) + 10 = 23
Attack 25: 1d20 + 10 ⇒ (16) + 10 = 26
Extra d4: 1d4 ⇒ 3
GM Infinity
Godric, you feel a strange surge within Blackrazor as it connects again and again...but also something from the Raven itself. Blackrazor snickers in delight as it devours a part of your essence...
Necrotic: 2d10 ⇒ (1, 7) = 8
...but at the same time, your divine smite seems bolstered...
Yet more smite dmg: 1d8 ⇒ 2

Lyraath
Lyraath moves into the corner of the room and continues to try to tell the others - especially Godric and Molok - where the creature is.
Then, clenching his teeth, he builds up his thoughts into a tightly woven bundle of anger and fear. He compacts it down into a small mote and then adds more to it, growing it before compacting it down tighter. Again and again until the thoughts are ready to explode out, he finally wraps it up in the fresh memory of falling towards the boiling mud and launches it into the mind of the creature.
6 point Psychic Assault: Psionic Blast. Wow, just saw that there's no attack roll nor save. It costs a good amount of points and is single target, but wow. Checked and Jeremy Crawford confirmed that's intended.
Psionic Blast damage: 6d8 ⇒ (6, 5, 2, 1, 1, 5) = 20
GM Infinity
Shoulder and knuckle, 2 points make a line. Before Lyraath stepped into the darkness, Narnel sighted down the line of his arm like ironsights, letting loose a stream of fletchery. One lets out a *crick*, the disappointing sound of arrowpoint on stone, but the other is an obvious hit to flesh...
...and the creature was not happy.
Lyraath meanwhile keeps eyecontact mystically with the vampire, sighting it on some psionic plane between the stars. He finds a ripple in that strange space and quickly exploits it mentally into a direct burst within the creature...
...and the creature was not happy.
Back at the door, in the normal cavern light, the ground seems to boil and churn briefly before exploding into a wave of rats, one after the other!
GM: 1d4 ⇒ 1

The swarm moves up and over Urth, probing every cranny...

14 more total dmg Molok, with a further 8 to your max hp. Some lucky rolls...Molok becoming a vampire is an interesting thought.

Urth Stormraven
Peering into the magical gloom, Urth attempts to touch the shape with his skeletal hand.
Chill Touch: 2d20 + 7 + 1d4 ⇒ (3, 2) + 7 + (4) = 16 13
if hit, necrotic damage and can't regain HP: 2d8 ⇒ (8, 3) = 11
Molok Granite Fist
Feeling his life force slowly being drained from him, Molok once again attempts to drag the vampire into the light of the corridor. He also shrugs off some of the vampires damage and centres himself to bring back some vitality to his life force.
Stone's Endurance: 1d12 + 3 ⇒ (3) + 3 = 6
Second Wind: 1d10 + 7 ⇒ (5) + 7 = 12

LB Dam: 1d8 + 4 + 1d4 + 1d6 ⇒ (1) + 4 + (3) + (5) = 13
Godric Alden
Godric watched helplessly as his blade once again ate his own life force. Just his luck, with the amount of undead in these caverns he wouldn’t have been surprised if he ended up doing more harm to himself than his foes.
Eldritch Blast: 1d20 + 7 ⇒ (1) + 7 = 8
Dis: 1d20 + 7 ⇒ (17) + 7 = 24

So he fell back on the tried and true method of channeling explosive force until the foe stopped moving.
Lyraath
Lyraath tries psychic hammer and trying to pull her towards the hallway, out of the darkness.
Psychic Hammer damage: 2d6 ⇒ (3, 3) = 6
Strength save DC 15 or force damage and pulled 10 feet in direction of choice.

Str vs Lyraath: 1d20 + 4 ⇒ (13) + 4 = 17
A resounding *whomp* echoes in the chamber as Lyraath's telekinetic force manifests, but the creature is resolute and stays put. Lyraath alone can also see her wounds closing disgustingly as tiny worms seal the tears in her flesh.
She turns her eyes to Lyraath, mouth still firmly biting into Molok's neck, and swirls of colors seem to bleed forth from her pupils. The magic emanations cross the room through the darkness into Lyraath's body...
Lyraath, wisdom save vs charm DC 17
Meanwhile out in the hall, the swarm of rats continues to harry the river man...
