I've got a Kensai Magus, currently 18th level, who will be using Sense Vitals spell in a battle with a dragon.
What are some ways I can get Sneak Attacks in, besides flanking, against an Ancient dragon? I know about the Magus Arcana Prescient Attack already and will be using that too when I can. I also hope to use Gang-Up feat to help get flanking easier.
I'll be getting melee help from a 2-handed weapon fighter and an Unchained Rogue.
Also have a Skald and Wizard in the group.
Also looking for help on ways to overcome the dragon's spell resistance.

Sneak attack-
win initiative
have rogue take the Distracting attack and designate you as the beneficiary
smoke/fog/etc spell with an ability that allows you to see through said fog, be 10ft away from him and make your stealth checks.
Dampen Presence feat
SR:
Spell penetration
be an elf.
Alchemical or magic items that increase CL
Also, depending on which type of dragon, don't get hit by anti-magic field.

Feinting? Then you and the rogue both take the teamwork feat Feinting Partner so that you're handing off SA's to each other?
Can you get anyone to try and grapple the dragon? Its a longshot but pinning it will deny it it's Dex bonus.
I'm guessing making an ancient dragon Frightened is pretty tough... but Shatter Defenses?

Use obscuring mist or smoke or something similar to blind the dragon. Be prepared for a smart dragon to fly away from everything, stay out of melee range, and cast antimagic field.
How does one kill a smart ancient death machine?
Use calcific touch to reduce it's dexterity to 0 and completely immobilize the dragon (it will take a few rounds). You really only need to worry about spell resistance, as the touch AC of dragons is laughable.
That's the wizards job though.
What do you, the fighter, and the rogue do? Distract it.

People keep forgetting that dragons have Blindsense. Fog tricks usually don't work against them.
Blindsense lets a creature notice things it cannot see, but without the precision of blindsight. The creature with blindsense usually does not need to make Perception checks to notice and locate creatures within range of its blindsense ability, provided that it has line of effect to that creature. Any opponent that cannot be seen has total concealment (50% miss chance) against a creature with blindsense, and the blindsensing creature still has the normal miss chance when attacking foes that have concealment. Visibility still affects the movement of a creature with blindsense. A creature with blindsense is still denied its Dexterity bonus to Armor Class against attacks from creatures it cannot see.
