I found an inconsistency in the rule book.
In "Chapter 1: Overview," under the "Character Creation" section, in "Step 6: Apply Your Class" on page 14, it states the following:
"To determine your character's total starting Hit Points, add together the number of Hit Points she gains from her ancestry (noted in Step 2) and the number of Hit Points she gains from her class."
However, in "Chapter 3: Classes," under the "Reading Class Entries" section and the "Hit Points" subsection on page 42, it states the following (emphasis mine):
"To determine your character's starting Hit Points, add together the Hit Points she receives from her ancestry, her Constitution modifier, and the number listed in this entry."
As per the usual specific-trumps-general rule, I would think anything in the chapter on classes beats the general overview on character creation but that just seems like a weird oversight.

No dice rolls, no. It's always a set amount + Constitution modifer. You can still reverse engineer it though, because it's just the maximum number for the dice they had in PF1 (Wizards had d6, they get 6; Fighters had d10, they get 10; etc)
Yes, but I think it should be an option in the manual, just like they made it for the stat rolls.
