Helm of underwater action lists "Slot: helmet" (p. 378) while helm of teleportation lists "Slot: -".
Am I right to assume that these are both typos, that there is no "helmet" slot, and that all helms (other than spelljammer helms) take up the "head" slot?

Yes, this is just an issue of standardizing the terminology. Head/helm/hat/headband/phylactery should be the "head" slot, masks/eye lenses/goggles should be the "face" slot, etc.
That was gonna be my next question. ;-)
So the actual WI slots are: head, face, neck, shoulders, body, chest, belt, arms, hands, feet?
Re masks -- the "masks" in RotRL #2 were pretty much full head affairs, like executioner's hoods. I could see a case where these would take up both the "head" and "face" slots simultaneously.

And - (no slot).
