Other side of pond:
Catriana gets out of the pond and turns to her friends, distressed: "Eglantine's noble sacrifice should not be in vain and we should use the time she gained for us. One of us should go get some help from the village. The others should stay here and ambush the fey as they enter this world through the pond". Looking back at the pond she adds, sighing: "If only we knew why Yomi and Cyrus aren't here yet..."

"My Lady?"
Eglantine sounds astonished and slightly amused, well aware of her own fragility, but she responds in elven.
Her tone grows stern. "My Lord has injured my friends in an affair concerning only he and I. Before I claim blood debt, tell me how, in honour, we may end this duel. I will not smirch my Lord or myself in this matter."
She shakes her wet hair out of her face and swims further away from Flax. To O7.
1d20 + 8 ⇒ (17) + 8 = 25 - Athletics check to stay swimming
1d20 ⇒ 18 - Save v daze.

Recharge Howling Strike: 1d6 ⇒ 4. Bah. Who does a fey have to kill to get a recharge around here?
Ellessandra starts to answer, "He is not --," but is cut off by Flax's harsh, "Stay quiet, Mother. This is not your concern!"
Flax knifes through the water and catches up to Eglantine. Calling on his reserves, he brings his greatsword around once more, and Eglantine slips dying below the waters.
Athletics to swim twice: 1d20 + 12 ⇒ (17) + 12 = 29 1d20 + 12 ⇒ (3) + 12 = 15 Action Point. Auto-make Athletics to attack while treading water. Greatsword vs AC 1d20 + 14 - 2 ⇒ (14) + 14 - 2 = 26, Damage 2d6 + 6 ⇒ (6, 5) + 6 = 17
Drowning rules. In addition to her death saves, as long as she is unconscious underwater, Eglantine loses one healing surge at the end of each turn until she runs out, at which point she starts losing her level in hp each turn.
Forgot Yomi's save vs. dazed last turn 1d20 ⇒ 13
Yomi, seeing the blood darkening the water as Eglantine falls, attempts to catch her in the pond but fails to reach her at this time. 2 Athletics 1d20 + 5 ⇒ (9) + 5 = 14 1d20 + 5 ⇒ (2) + 5 = 7. AP for another. 1d20 + 5 ⇒ (10) + 5 = 15

Athletics Checks, 1d20+1=10, 1d20+1=16
Ok, that got you halfway to the bottom last turn. One more DC15 this turn to reach the bottom, or Eglantine if you'd rather.
| Eglantine Prakseda |
One of Eglantine's hands goes to the hilt of Flax's greatsword as the blade drives home and she pulls herself upright enough to look him in the eyes one last time, her bloody finger tracing his lips.
"I claim blood debt my Lord. Know that this will never truly end."
As her body sinks out of Yomi's reach, she is smiling.
-14 at start of her next turn. 2 surges remaining. Second Wind not used.

Cyrus has reached Eglantine, and needs to make one more DC17 check to tow her to the millstone at the bottom of the pond. Flax is returning to dry land, so initiative now matters only for purposes of whether Eglantine dies before being healed or stabilized.
Yomi Athletics check to reach Cyrus and Eglantine: 1d20 + 5 ⇒ (13) + 5 = 18. Succeeds. DC10 aid another to help Cyrus get Eglantine to the millstone. 1d20 + 5 ⇒ (11) + 5 = 16. Succeeds.
Eglantine, make save vs. death and save vs. ongoing, and lose a surge to drowning.
Yomi -- already moved.
Cyrus -- needs to make one DC15 (after aid by Yomi) swim check to reach the millstone towing Eglantine.

Sorry, I wasn't clear. You need to succeed once, but they're move actions, so you can try twice, which you've done. You can save your AP or use it for a third try this turn.

Eglantine is gripped by Yomi and Cyrus, who are working together to tow her down to the millstone while she yet lives.
Eglantine, make save vs. death and save vs. ongoing, and lose a surge to drowning. One failed death save. Please give complete status at the end of this turn.

Yomi, being the stronger swimmer, waits for Cyrus to adjust his grip on the drowning woman before attempting to tow her to the millstone.
Yomi is delaying in the hopes that Cyrus will Aid him in the Swim check.
Cyrus - swimming, DC10 Athletics to aid, DC17 Athletics to tow, DC15 to reach the portal alone. Each attempt is a move action, activating the portal is a minor.

Cyrus - one DC15 swim check needed to reach bottom, get two tries.

Cyrus is still trapped in the Feywild side of the pond.
Yomi attempts to tow Eglantine to the surface in time to resuscitate her. 1d20 + 5 ⇒ (11) + 5 = 16 1d20 + 5 ⇒ (9) + 5 = 14
But just as they break the surface -- seconds after Catriana, from the Riverbend point of view -- the cleric realizes it is too late.
Eglantine Prakseda has died in his arms.
