Luralyn, unless you have some way to ignore difficult terrain I'm not seeing, that was a 100ft movement (the green squares are difficult terrain). I adjusted your position to 60ft. of movement, though you can adjust if you'd like.
Janira lashes out with her whip, trying desperately to knock the fearsome ax from her pursuer's hand. With a yelp, the sting of leather causes the beast to drop its weapon, and Janira uses that opening to step back five feet.

As soon as she does, Fareth quickly reads the scroll, causing the plants around the beast to writhe and grasp at the minotaur, wrapping themselves around its huge legs!

The beast rips itself free of the twisting plants, then picks up its weapon and bellows in rage!

Before I commit to something reckless here- Are we going by the visible width of the river at any given point, where it seems to be 5' across to the east of where I am currently, not even 5 up north, and up to 12 in places, or calling the DC to clear it a flat 10 regardless of where you are?

If you're willing to go by a less direct route to get to a narrower spot, it seems only fair to call that narrow spot DC 5.

Quasar frowns, seeing the minotaur has stepped just too far away to be properly battered from this side of the river. Giving it a glance, and seeing it at least looks a bit more narrow than the pit in the cave in parts, and leaps across, hoping to at least catch the minotaur's attention, and hopefully not be flattened for her efforts.

As she's airborne, she pulls together a pile of loose stones from the riverbottom to send upwards towards the already clearly enraged brute.

Reshu readies to disrupt the minotaur, should it approach, while Quasar seizes the moment and fires, connecting squarely!

"Foul beast, you have no business here! Flee, for you shall surely be destroyed!" Janira's words don't seem to hold the same power they did during the battle with the snakes, her fatigue evident. Still, she draws a tanglefoot bag and hurls it at the minotaur, covering it in rapidly-hardening tar before stepping back again.

The minotaur bellows in rage as its pursuit of Janira is further stymied as the hardening tar glues it fast to the ground! It begins to furiously scrape the goo off of its legs, bellowing with triumph as it breaks free!

Reshu bursts into laughter at the minotaurs attempts to free itself. "Well no point in waiting around. That beastie is pretty well stuck!"
With that he rushes across the log still holding onto the spirits power for when his allies engage the minotaur. You said the log looked difficult to cross so I'm assuming I need an acrobatics check. If I pass Reshu again readies his stardust ability for if the minotaur and his allies find themselves in melee range of each other.

Somewhat surprised that he made it across the water unharmed, and dry, Niocholu races toward Janira, skirting the patch of grabby plants. "Hmmm." he ponders aloud, "Niocholu probably should have grabbed bow rather than pike, yes? Bow more useful at moment."

Fareth start carefully making her way across the bridge, moving half-speed.

Unfortunately, while trying to navigate around Drilla, she steps off the log and falls in the water. She attempts to swim toward the opposite shore.

Coughing up water, she calls Drilla to her.
"Drilla, heel!"

Fareth slips, and with a splash joins Luralyn in the water, emerging waterlogged but none the worse for wear on the other side as she calls Drilla to join her. Reshu slips and falls on the log, but is able to catch himself and get back to his feet (he had a standard action left, which I'm assuming you'll use to stand). Niocholu leaps, barely clearing the rushing water. For a moment, he nearly falls backwards into the river, but a few panic-induced windmill arm motions later he regains his balance. Luralyn easily swims across, emerging on the other side firing an expertly placed stone at the minotaur. Quasar fires at the minotaur once again, and once again strikes true. I'm picturing pushing someone whose feet are held in place, and I think the most likely outcome would be falling over. I'm going to give it a Reflex save to avoid falling prone.

The plants holding it in place prevent her from pushing it away, and it manages to keep its feet against the force of her blast.

The minotaur bellows and rips itself free of the grasping vines. Tired of being entangled, it stalks toward the edge of the writhing plants. It can't quite make it out, though, and it (unbelievably) is once again grasped by the vines!

To make matters worse, Janira once again expertly disarms the poor creature. What a terrible, horrible, no good, very-bad day this minotaur is having!

Am I correct that Niocholu cannot ready both a free action (to Rage) and a standard action (to attack)?

That's my understanding, that it's one or the other. That said, you could enter a rage now, then ready the attack for later in the round.

Luralyn slips inside the creature's reach, ducks its flailing arms, and delivers two rapid blows to its solar plexus; with each jab, his long hair flares backwards in a sudden burst of wind.

Quasar takes a step back, visibly relieved the people who actually look like they can take a solid hit have interposed themselves, and leans over to Janira for a quick, "You OK? I'd thought you were just going to keep running back towards the city until this jerk gave up."
She then once again pulls together the stones for another oversized blast.

Luralyn lands two quick punches, bang bang, causing blood to trickle from the beasts large nose. Dazed and distracted, the beast makes no move to dodge Quasar's blast, which hits it squarely between the eyes and half-burying it as it falls unconscious at the approaching Fareth's feet. Out of combat! The poor thing never even got to attack.

Seeing the creature fall, Janira puts her hands on her knees and takes a deep breath. Straightening, she says "Are any of you hurt? I'm so glad you showed up when you did! I was just about out of ideas." She looks around, seeming to recover her excited spirit. "So, what'd you find in the cave? Did you see any gillmen?"

Quasar nods, straightening her clothes again. "Just the one... well, the one who was alive. Religious sort, seemed friendly and harmless. There were also a number of... corpses returned to life by a dhampir with an oddly convenient note of confession on his person... and a bit more exploring we still need to get to, but, well, obvious priorities." She gestures to the minotaur.

Neither did I!There was nowhere where I wouldn't have been entangled or provoked an aoo.
Reshu hurries over to Janira excitedly. "Yes these caves are a place of religious significance to the gillmen! Sacred waters and such. It was an Arodenite shrine at one time apparently!"
Taking a closer look at Janira's wounds he calls to Fareth, "Friend Fareth I think Miss Janira may need a poke from your...Oh! I mean her healing stick."

A chill wind blows up the Eastern passageway, though it carries none of the stench of the Western passage. After about 20 minutes of travel, past many branching passageways that either narrow to be too small to pass or obviously loop back toward the main cavern, the tunnel broadens to reveal a shallow river that bisects an immense tunnel stretching to the north and south. To the east a small passage exits the chamber at a steep incline. The river through this part of the cavern ranges from 10 to 15 feet wide but is only 1 foot deep, and it flows languidly at an almost imperceptibly slow speed. Treat squares with water as difficult terrain.
Across the water, three small blue figures stand next to a large (compared to them) segmented insect with way too many legs. When they spot you they all point and begin shouting in a strange language as they draw their knives!

Rushing into the cavern Reshu gestures at the insect-like creature wreathing it in light. Reshu uses his Stardust ability causing the creature to shed light, and take a -1 penalty on attack rolls and sight based Perception checks this round.
"Mites! Watch out they're spiteful little buggers." He shouts back to his friends.
