Roshan smiles to see that the girl is in good condition, then blinks up at the old woman's ignoring of her. She clears her throat and stands on tiptoe, but comes short of waving her hand to get the old woman's attention, at least.
"Yes! My friend has a gift for you! That bottle! But we also have medicine, and Ollystra Zadrian presents her regards. May we come in, ma'am?"

"Ahh yes I see. And a good vintage." The old woman snatches the bottle from Faeria's hands. "Medicine from Ollystra of the Shinning Star. Oh what a nice lady she is."
She looks the gnome up and down. "You may come in dearies, but only for a minute I am very busy as you can see." She waves her hand in the direction of the numerous kids in cats running around the house. "Make sure to wipe your dirty feet before you come in."
You can spot broken blood vessels in the eyes and
nose, shaky hands, glassy eyes, and a fine sheen of sweat
on the woman.

Roshan asks where the medicine should be set down-- on the hopes she might be able to put there herself, and check what sort of other supplies are there for the children's injuries. She asks one child if everyone gets sick often and whether they use a lot of medicine.

There are a lot of you.” She gives the group a puzzled look. ”Why does it take so many Pathfinders to deliver this small package?” As Kagar, Fellstone and Theoduin move to come in the house she begins to close the door a little using it a protection. ”Would you men wait outside please. My house it full as it is.”

Fellstone gave a shrug at woman suggestion that they wait outside and was about to step back into the courtyard when he remembered that they were here not only to deliver the medicine but also to look for anything suspicious. Asking the three of them to wait outside might be construed as suspicious.
"We're small, don't take up much space." he says, eyeing the woman and not stopping from entering the building. "Well, excepting for Kagar but with Faeria sitting on his shoulders it saves space."

As the two monks push there way thru the doorway. "Well if you insist!" She gives the Half-Orc and Dwarf a disapproving look.
Answers to everyone's questions are below in the spoilers. If I missed anything let me know.
You do not notice anything other about her then she has the look of an alcoholic. You do notice a half empty bottle of liquor in the sitting room.
”Her dearie, just give it to me I’ll put it away.”
When you try to talk to the children she shoo’s them away before you can get a word in. You get a sense she does not want you to talk to the children.
After your question of what she does with the medicine. The old woman huff’s with impatience. ”The sick are keep upstairs so they don’t the rest of the children sick.” You can hear coughing and wheezing coming from the stairwell just outside of the sitting room.
She leads the group, minus Theodiun and Hugo who are still outside, to a comfortable sitting room just off the main entry. She shoo's off any cats and kids in the room. "Please take a seat. I will get us some tea." She turns to walks to the kitchen, which you can just see thru a archway to the back of the house mumbling something to herself and shaking her head.
"Half-Orc's and Dwarves have no manners! Can't believe Ollysta sends a dirty little gnome to deliver my medicine."

Kagar shakes his head at the woman's mutterings. "Given her manner, she's xenophobic and given to much alcohol. Not truly anything dire. Just annoying traits. I should like to speak with the ailing children. Leave you to this woman."
If no one objects, Kagar will excuse himself while Baltwin is in the kitchen to head upstairs, following the sounds of the coughing.

Faeria likes one palm and then uses it to flatten Roshan's hair. "There." She says approvingly. Her eyes not exactly in focus. "Other than that cowlick you look a clean enough gnome to me." the wild-looking forest woman adds.

Auntie Baltwin slowly walks out of the kitchen behind Fellstone and Theoduin bellowing at them as she does. "Watch out!! Those cups and plates are very old. Put them down here not over there!"
The old woman takes a seat with her cup of tea half full and tops it off from the bottle you gave her. "I hope this is as good as you say." She takes a sip puts her cup down then stares at the Pathfinders and says. "So what is the real reason you come to my house today? Surely not to have tea?" She has a crafty twinkle in her eye like she is ready for a battle of wits with the group.

You let all your kids out to play every day in the streets so I had plenty of friends--well not exactly friends but.... You drink a lot of tea, and like cats, and can tell good people from bad just by looking at 'em. So I guess everything's okay."
