I factored in soft cover(+4 ac) and grappled(-2 AC from Dex penalty) already. Generally I don't make this kind of mistake.

you get soft cover for grappling? I didn't see it mentioned in there. I keep losing posts to this backtracked too far error, it's really ticking me off. I realize you mean soft cover vs reach. I didn't think it would have applied to grappling for some reason.
I'd definitely have popped inspiration if I knew I'd be at 16 total, considering Leonard is in a bad situation.

Some of the creatures wounds heal. 20 dmg done. The creature floats over to Vincent (5 ft step) and tries to bite him, then slam him with its viscera.

The bite does not penetrate his toughened skin and the viscera slam hit the wall instead.

The creature does not see the tentacled thing sneaking around and it gets to Leonard safely!

How are you getting +5 for UMD on your weasel? UMD is based on Cha?

Seamus sends two arrows into the creature, wounding it badly, even if his arrows don't seem fully effective. The arrow head begins to slowly push out. (47 dmg done)

It really doesnt like being the target of arrows. It floats towards Seamus (5ft) then bites and tries to grab him with its viscera. Seamus ducks the bite but the coils of viscera grab him and begin to squeeze.

Seamus does not manage to stab through anything but Vincent on the other hand manages to pierce her cheek, injuring her badly. The weapon does not seem to be fully effective though. 59 dmg done

Vincent, you can flank with Howard but not Seamus. Either way, you still hit.

Actually, it looks like I've got 7 con damage.

Leonard rises silently, grasping his sword once again. It bursts into flames the moment he touches it, causing long shadows to writhe in every direction. Should've made sure I was dead.

Leonard's blade cuts into the creature but surprisingly it is not harmed by the flames.

"Yes, I supposed I should have finished the job.Time to correct that now." The cut inflicted by Leonard heals completely. 59 dmg done to her

It then lets go of Seamus shifting towards Leonard (5 ft step) and snaps at Leonard, and tries to grab him with its viscera again.

Leonard tries to parry the attempt to grab him with its viscera, but the blood loss makes him too slow, as he is grabbed once more. He feels the world go red, then black, as he collapses in the coil of her viscera.

The party hears something distinctively audible crack, as blood dribbles out of his mouth, as black veins begin to creep up his face.

Vincent, his mind bent on vengeance, runs the creature through the back. "Noooooo!" It gives a horrifying screech as Vincent stabs it in its heart and it disintegrates into dust.

Combat over heh.

With that screech, the party feels a cold wind smelling of decay blow from behind them, as if something was making its way up the stairs. From door 5

Now what?

Vincent drags Leonard into the room Howard mutilated bodies in and tells the others to close the door while he sets to healing Leonard again as the thing hands him the wand and slithers back into his coat.

Not sure total vs current so roll as many as you need.

Entering the room the party sees that the decapitated corpse is now a fine pile of dust. The mnemonic vestment and chain shirt the body was wearing now lies on the bed.

You have a round before whatever that was coming up the stairs catches up with you.

Ending his song, Seamus stands dumbfounded for a second, watching Vincent's actions. "Retreating back in the room and locking the door? I like it. Maybe whatever is coming will just go past."

"I hate doing this, but he wont be happy if we leave it here." Seamus runs forward and picks up Leonard's sword, holding it by the pommel, he drags it into the room.

Vincent tosses Seamus the wand to work on Leonard while he braces the door. He also borrows Leonard's wand and casts shield on himself. Or lock if it has one?

The door bursts into splinters, as if sundered by a heavy blow.

A disembodied female torso flaps through the air on batlike wings, her fanged face a hateful mask can be seen through the door.

Vincent knowledge religion: 1d20 + 10 + 1d6 ⇒ (15) + 10 + (6) = 31

Vincent recognizes her as a Manannagal. They have 10 DR good/silver. And can grab with their claws and drain blood.

Most importantly:

Separate (Su) During the day, a manananggal looks like a living human woman. She does not detect as undead during the day, but is still an undead creature. At night, her upper torso rips away (this is a full round action that occurs at sunset), leaving her lower torso behind. Her lower torso is helpless, but her upper torso gains its fly speed and natural attacks at this time. The upper and lower portions share the same pool of hit points (despite any intervening distance), and if the helpless lower portion is damaged, the manananggal is immediately aware of the attack. Since manananggals can be destroyed by damage to their lower bodies, they prefer to hide their lower torsos when separated. A manananggal must return to its lower torso and reattach to it (a full-round action) within the hour before sunrise—each round a manananggal remains separated after sunrise, it takes 1d6 points of damage until it rejoins its lower torso or it crumbles into dust.

Vincent recognizes the lower torso as the sawn off torso on the bed.

Vincent shouts at them to hack the lower body to bits while he tries to keep it from entering, hoping that Leonard can help with that also now that he is conscious. He studies the upper torso and strikes. Dm want to roll whatever healing Seamus and I managed before door burst open? Not sure if Howard could have helped in the time before also, maybe cure serious if he has one?

Doesn't the slam miss since my AC is 25? Also, pretty sure my maximum HP is currently 8, since I should have 11 points of con damage. Let me know if either of these things is incorrect.

Leonard is barely alive, it's amazing that he's somehow conscious. He raises his sword with some effort, concentrating intently on it. With a gasp, he brings it down on the severed lower torso.

Swift action spend another arcane pool point changing to +3 and flaming instead of keen and flaming, thus bypassing DR silver. Coup de grace the lower torso.

Damage - dex penalty + flaming + bardic performance *crit: 1d6 + 5 + 1d6 + 2 + 1d6 + 5 + 2 ⇒ (6) + 5 + (4) + 2 + (3) + 5 + 2 = 27

From the rat fight, more then a minute, so your shield wore out. AC was 21 since you heard no noises it was not refreshed before you got jumped.

Your base hp is 44. Ability damage: For every 2 points of damage you take to a single ability, apply a –1 penalty to skills and statistics listed with the relevant ability. So you took 11 points of ability damage, counts as a - 5 penalty to your con, which gives you 5x6(HD) less hp.
You're actually very lucky to walk get carried out alive.

As Leonard brings down his blade, he hears the creature in the doorway give an unholy screech of pain as it looks seriously weakened. 41 dmg done

Seamus strikes the arrow straight into her lower torso,"Arrrrrgghhh!" it screams one last time and there is a flash of bright light, then both the creature and the torso crumble into dust.

The room goes still, except for Leonard's laboured breathing.

Searching the room, the party finds in the drawers two scrolls of sepia snake sigil, a scroll of see invisibility, a composite shortbow (+3 strength rating), 20 arrows and 329 gp.

One of the bedside tables holds five empty glass phials and a notebook penned by Lowls. Vincent identifies the empty glass phials once contained a sleeping drug.

In the notebook penned by Lowls with a lengthy series of notes about his suspicion that Ulver Zandalus was the key to discovering Neruzavin, the lost city in Casamaron that holds three Star Stelae similar to those on Thrushmoor on the hill.

There is an earlier entry identifying Leonard by his name, race, gender, height and weight which was referred to when administering the exact dose of a special sleeping drug provided by a person called Miacknian Mun.

Leonard sits down on the bed after Seamus finishes off the monster, lying down right next the now bloodied corpse. Oh dear, I am very tired now. He closes his eyes and inhales deeply, laboured breath sounding raspy and painful.

Leonard sits up, putting on a brave face, albeit one with ugly black veins popping out of it. Well now, not about to let a little blood loss put me down, am I? No, that would be too easy. Angiliath flashes out of its scabbard, although a bit more sluggishly than usual. Wands are used, and the room is searched. The wand that can fix us is empty now, a mere stick, but don't worry. We'll manage.

Later

Leonard keeps one hand on the wall at first, but after a moment he removes it, trusting his feet to keep his balance. You're outnumbered, and probably outclassed. If you don't drop your weapons right now I'll kill your whole family. Leonard's left hand turns an ugly red, a color far more pronounced than usual because of his pallor. His sword doesn't burst into flames, but it does dart forward, still quite fast, still quite deadly.
