There are still three children unaccounted for: Mikra, Jurin, and Hollin. You know from the halfling that Mikra and Jurin made it to this level but Hollin was felled by the kobold's paralytic poison and is most likely still held captive on the second level.
You open the double doors to the East: A ten-foot-diameter pit occupies the center of this chamber. Directly over the pit a long chain feeds through a complex pulley system before disappearing into the depths.
As you survey the room each of you notices the scaly forms of kobolds attempting to skulk about in the darkness. Noticing your notice they scream out in frustration and attack! Savram screams out in horror, Don't let them take me back! and runs out of the room through the doors to the South.

Moving forward I will use the first initiative roll for the entire combat sequence so you don't need to roll each round.

To everyone: Apparently we lost initiative. Might want to go first sometimes. Would like to roll iniiatve individually. I realize it takes time and slows game but might be very important in melee. I might need to bomb first or second. Chris I agree with your thinking but may affect play. I understand the wish to move the game forward. If no player rolls for iniative in 24 hours they are last to act?
.Savram is my buddy, just because I was useless and paralyzed does not mean he should run. If he runs he still has the dagger I gave him.

Jalriel gets a jump on the fierce looking kobolds but promptly spoils his attack by slipping on some discarded kobold trash. He ends so out of position that he won't get to attack till the end of next round!
The kobolds attack the party, trying to flank at every opportunity. Jalriel is hit once for 6 points and must save vs DC 16 poison or take an additional 1d4 ⇒ 2 points of damage and 1d4 ⇒ 1 points of Dex damage from the bitter burn of poison!
Azzakan, caught flat footed, is hit twice for a total of 9 points and must make TWO DC 16 poison saves or take1d4 ⇒ 31d4 ⇒ 3 worth of additional damage and 1d4 ⇒ 31d4 ⇒ 1 of Dex damage.
Daedeloth and Variel combine their attacks and are able to drop one of the killer kobolds!

A character who has not yet acted during a combat is flat-footed, unable to react normally to the situation. A flat-footed character loses his Dexterity bonus to AC and Combat Maneuver Defense (CMD) (if any) and cannot make attacks of opportunity, unless he has the Combat Reflexes feat or Uncanny Dodge class ability.
Characters with Uncanny Dodge retain their Dexterity bonus to their AC and can make attacks of opportunity before they have acted in the first round of combat.

Azzakan drops his crossbow and quickly pulling his short sword out stabs a kobold in the leg; he maneuvers into a position that foils the kobold's attempts at flanking the party members. Daedeloth and Variel both score cuts on their foes, but the scaled creatures remain standing.

Intimidating is a standard action so you could do that or attack; not both.
It would have caused the second attack on Jalriel that round to miss. I'll let that action (and roll) stand for your action this round if you like; or it can stand for last round and your miss for this round. In which case Jalriel take off the 2nd attack from last round.

Without my so called Mage armor and minus 1 because of dex loss, I still prepare a bomb. Nothing else I do works anyways. I shall meet my demise doing what makes me the happiest...building a bomb.
I think that's a single round action (bomb) so you can roll for it this round. Make sure to adjust for you decreased DEX and tell me who you are throwing it at as all the kobolds are in melee and the splash will probably catch some PCs.

Still need clarification if Azzakan decided to intimidate the previous round or not. That would put him melee missing this round with a potential bomb to throw next round. Also, what's everyone's current AC with the DEX damage?
