1. I know that if you purchase a rebuild with achievement points, you're not allowed to change your ancestry, heritage, or background. Does this change if you buy a boon for an ancestry, heritage, or background?
2. If you purchase a boon that allows something specific to a book & you're asked to document it, do you need to show the specific book, just the boon document, or both?
3. There is a boon called "Secondary Initiation." It allows you membership in an organization other than the Pathfinder Society. Is that enough to take dedications from the Knights of Lastwall, the Hellknights, the Firebrands, & so on? I really want my Warpriest to be a member of the Knights of Lastwall but can't figure out how. Am I badly over-thinking this?
4. General Feat -- Adopted Ancestry. Does this allow you immediate access to feats from your chosen ancestry? For example, my Catfolk adopts Humans, so at 3rd level he takes General Training for Incredible Initiative, & at 5th he does it again for Fleet?
5. 1 of my characters is registered here as having more games than I have Chronicle sheets. Is this an issue?

3: Kinda, but here's an expanded answer for 3:
ownership vs access vs requirements
For example:
Lastwall Sentry: Source is World Guide, which is part of core assumption, so no ownership is needed. Access requires that you're from the eye of dread region, so you need to pick that as your home region or buy the world traveler boon. Prereqs require membership in Knights of lastwall which is fulfilled by secondary initiation, and Shield Block.
Knight reclaimant: Character Guide is needed for ownership, secondary initiation fulfills access (And prereq of "member of knights of lastwall of knight rank), and you need stealth and survival as skills.
Hellknight armiger: World Guide (no ownership required), access: You are from old cheliax (so home region or world traveler boon), prereqs: Member of hellknight order (through secondary initiation) and trained in heavy armor and specific weapon.
Hellknight dedication: Requires that you have hellknight armiger dedication, and ownership is from character guide. Membership and "passed hellknight test" requirements are met by the same secondary initiation boon. Signifier is the same, except also requires spellcasting class feature.
4. You don't get an ancestry feat at level 3 so you can't take general training to get incredible initiative, but you can use your 5th level ancestry feat to take general training: Incredible initiative, or fleet.
Note that for PFS, adopted ancestry is allowed to pick from those ancestries that are either common, or "always available to all pathfinders", so you could pick human, or Catfolk, but you couldn't pick Android even if you bought a boon for android.
5. Depends.
Did someone accidentally report games to your character that you didn't play? Then it's not much of a problem, and if you don't know the GM, it's hard to fix. You could email orgplayreportingerrors@paizo.com with your player and character numbers and list of "additional" games and ask them to remove them (or move them to the correct character if you played the games with a different PC, but in that case try to contact the GM first)
If the problem is that you DID play the games with this PC but you didn't receive chronicle sheets, then you need to contact the GM. If the GM is unresponsive or you don't know how to reach them, contact your local venture officers to help fix the issue. If it was from a con, contact the con organizers.
