So Astrazoans can transform into any small, medium or large ancestry and maintain that form more or less indefinitely. How does that affect gear? A Skittermander and a Dragonkin need wildly different clothing/armor, for an extreme example? This also has mechanical implications at low levels, since large gear has twice the base cost.

Well, in 1e there were a couple of different armor options that could either change size with you, or be rapidly adjusted in order to fit different sized wearers (obviously the first option is more expensive). Presumably we'll get at least some of those options in the Core.

Does large gear still cost double in SF2E? Genuinely asking because that kind of thing is generally a pretty small rule and I didn't read the playtest book that thoroughly.
It's possible that thanks to SCIENCE™ all armors can just adjust to fit whoever, or it's something you can do with crafting with minimal effort, and that rule isn't a thing anymore.

Large PCs should generally be able to acquire equipment sized for them without having to pay a cost increase; these characters come from cultures that regularly craft appropriately sized gear for them to use.
Unless Starfinder 2E deviates from this, equipment for large PCs costs the same as medium PCs.

It's just odd because the Dragonkin's high level feat to briefly become huge does call out your gear changing size, so I can't tell if this is a deliberate omission. At the same time, there's a ton of combat shapeshifting feats for Astrazoans, several of which are dedicated specifically to changing size, and those don't work well if you lose your gear as you shapeshift.

There is at least a couple astrazoan feats that seem to indicate they need you to be in no or light armor but otherwise nothing really calls that out. A lot of the armor in starfinder calls out using a lot of force fields to help bolster the armor so may just be a case in other than the couple feats where it is called out the assumption astrazoans buy armor designed to auto adjust to their forms.
Given how freaking weird the various body shapes/sizes of starfinder player races is going to be just hand waving it probably does make a lot of sense.
Like in theory armor that fits a barathu can work on a bantrid so that gear has to be pretty darn flexible/adjustable.
