From what I understand Charon keeps himself veiled in mystery on purpose, so we may not know. Also worth remembering is the Book of the Damned's proviso that Charon's visible form may not be his true one, either, and that he might actually be some kind of monstrous, eldritch thingy that dwells within, or as part of, his seat of power in The Drowned Court.

It is notable that PF1 Bestiary 6 has cover art similar to the art from book of the damned, and is the bestiary that contains stats for Charon (and there isn't anything else in the bestiary that would look like that).
Also, both covers have the same boat in them (in the background in book of the damned, being paddled by charon on bestiary 6.
He also looks like that in his entry in the Book of the Damned

That is 100% Charon on the cover of Book of the Damned. We wanted to have the cover characters from the softcovers get together for a class photo.
AKA: Under his hat, he looks like a supernaturally old human man.
Whether or not he's wearing his hat or dressed in a cloak depends on his mood. He's not like a superhero... He can change his clothes. :P

Very interesting that he would look specifically like a Human.
New conspiracy theory (yet another) about Rovagug, Androffa, Earth and Golarion (and Humans and Charon) blooming.
It could also just be that humans look like Charon (and the various other demigods and deities). The gods all look humanoid, so you either have a situation where the gods created humans in their own image, or the gods changed to resemble what people expect them to look like. The vibe I get from the Lost Omens setting is that the former is the case.

I believe there are several deities in the setting who look like the people who look at them / worship them. Abadar and Desna come to mind.
Which is why I found Charon's appearance possibly telling.
Of course, since it is pure conspiracy theory, any serious, or better official, debunk will just go to show that my far-fetched theory is the real secret truth ;-)

I've always worked under the assumption that even if it's not included in their stat blocks, that the appearance of almost all demi-god level creatures is at least somewhat mutable, whether by their own whim or will or by the perception of those looking upon them.
But then I also give *all* dragons the change shape ability, not just metallics, because I think that's just a better narrative tool.
