I wouldn't allow you to try it in that order (lich and then fiend), but I would allow you to become first a fiend and then a lich.
The fourth apotheosis replaces your race and the half-fiend template. You need the half-fiend template for it to work.
But the half-fiend template says:

“Half-fiend” is an inherited or acquired template that can be added to a living, corporeal creature with an Int score of 4 or more. A half-fiend uses all the base creature's statistics and special abilities except as noted here.
When you become an undead the half-fiend template isn't applicable anymore, so you fail the requirements for the fourth apotheosis.
About the original question:
A lich isn't slain the normal way. You need to destroy its phylactery to permanently kill it.
Probably your infernal patron has the power to do that if you fail your ritual. If he would do that depends on the patron and how good a servant you were.
