I have a couple:
The Othaos Aeon. The concept is sound (a combination of light and darkness themes) but the picture looks rather like a black and white donut with arms.
The Qlippoth lord Isph-Aun-Vuln: her stats and description are actually pretty scary, however she looks like a giant meatball with a mouth and eyes, which I find impossible to take seriously.

meh almost all of them. Paizo has commissioned some excellent work, and by no means am I attempting to downplay the artistry, but I usually prefer the visuals of my own mind (surprise surprise). If as a GM I ever use creature/NPC art during sessions, I will almost always search the web for other artwork with a similar feel for what the creature is, but with a better aesthetic. You can usually find something by browsing Pinterest, deviant art, or google.

Modrons might of been cool since you could imagine them being pulp fiction sci-fi robots ala lost in space or the blackhole.
Instead they look like someone decided to just use the dice as direct representations of the monsters (I'm nearly positive this is exactly what happened).
This is the original artwork for them (doing a search these days results in slightly less goofy looking monsters)

Not a "monster" exactly, but when the Oread first came out I had a hard time accepting the idea that accompanying illustration was one of them.
Mostly because of the "Oread" name, really : it just didn't fit, at all.
I would have been okay with the pic had the race been called anything else.
As it turned out later art was much more adapted, a bunch of which I really like.
Still a bit miffed that we won't get actual oreads, and that this was the first canon, iconic illustration. Ah well.

The B1 Cloud Giant never did much for me.
I really like the little written description in italics at the beginning of PF1 Bestiary entries. I think it's a pity that they didn't keep those for Starfinder Alien Archives and (I think) they've abandoned them for PF2. Having a nice one or two line way to describe a creature was really useful.
