Does the AC bonus for Wisdom kick in right at level 1?

When unarmored and unencumbered, the monk adds his Wisdom bonus (if any) to his AC and his CMD. In addition, a monk gains a +1 bonus to AC and CMD at 4th level. This bonus increases by 1 for every four monk levels thereafter, up to a maximum of +5 at 20th level.

Does this AC bonus apply to CMD and touch attacks also?

Yes.

Note also that a Monk can still benefit from an armor effect. Bracers of Armor work, as does the spell Mage Armor. The spell Magic Vestment grants clothing armor as well. Just note these things don't stack with each other.

Yeah, first level, with further bonuses at 4th, 8th, 12th, 16th, and 20th, as mentioned in your post.

only vs incorporeal as far as i know
