do all of these fighting defensively bonuses stack?
fighting defensively- -4 attacks, +2 dodge bonus to ac.
acrobatics 3 ranks- increase dodge bonus to +3.
two-weapon defense- +2 shield bonus to ac.
crane style- attack penalty reduced to -2, +1 dodge bonus to ac
crane wing- +4 dodge vs single attack
crane riposte- attack penalty reduced to -1
total- -1 attacks, +4 dodge bonus to ac, +2 shield bonus to ac, +4 additional dodge vs single attack.

If you're building a character that is going to use Combat Expertise a decent amount (and if you're going the Crane Style route, you might as well go whole hog), don't forget about Threatening Defender. It's a trait that reduces the attack penalty for using Combat Expertise by 1.

Combat Expertise is a Dodge Bonus. Dodge Bonuses from different sources stack (Fighting Defensively and Combat Expertise are two different things).
EDIT: Note that you cannot use Combat Expertise when using Total Defense, though. Combat Expertise requires that you make an attack (like Fighting Defensively) and Total Defense prohibits making any attacks.

and if you are going to go so much out of your way, you may skip a few feats if you simply use a madu:

Wait, does that mean a halfling using Desperate Swing to attack while using Total Defense can also use Combat Expertise? I thought there was a special clause somewhere that said Combat Expertise and Total Defense don't stack.

No, Combat Expertise says it can only be activated when you make an attack. If you have some way to actually make an attack while using the Total Defense action, then you could activate Combat Expertise with that attack.
Once per day, you can make a single melee attack while taking the total defense action. You take a –4 penalty on attack rolls when making this attack. You also gain a +4 bonus on critical confirmation rolls made while fighting defensively or making an attack of opportunity using this feat.
You can only choose to use this feat when you declare that you are making an attack or a full-attack action with a melee weapon. The effects of this feat last until your next turn.
So, once per day, as a halfling, you can stack Combat Expertise with Total Defense when you make that single melee attack you are allowed.

I'm not so sure...
My read on Combat Expertise is "when you declare you are making an (attack or full-attack) action..."
It is well established that the attack action is a specific standard action, which is very definitely not the total defense action, which would make this interpretation incompatible with Desperate Swing.
Further, if it was "making an attack (or a full-attack action)..." then you could declare combat expertise on an AoO, which seems unlikely.

A creature can also add any circumstance, deflection, dodge, insight, luck, morale, profane, and sacred bonuses to AC to its CMD.
Well Dodge bonuses stack and they already apply to CMD, so you get 1.5 times your dodge bonus from fighting defensively/total defense to your CMD.
So say they have Blundering Defense and Uncanny Defense and are fighting defensively they'd get:
+4 Dodge to AC, +6 CMD, and +2 Reflex just from Uncanny Defense and Blundering Defense.
EDIT: With Crane Style and 3 ranks acrobatics that stacks to:
+6 Dodge to AC, +9 CMD, and +3 Reflex
