This has come up in a few threads, and I've been discussing with my GM. The main question here is, how often can I get my sneak attack bonus dice in a round, if I'm concealed with something like Greater Invisibility?
Does the sneak attack damage still apply to just the first hit, or to all successful hits in that round? Please provide book pages or verbatim on this, to avoid confusion.

You get your sneak attack damage on every attack that qualifies. So if you are using Greater Invisibility, or are flanking your target, you get to add sneak attack damage on every attack you make.
The rogue's attack deals extra damage anytime her target would be denied a Dexterity bonus to AC (whether the target actually has a Dexterity bonus or not), or when the rogue flanks her target. This extra damage is 1d6 at 1st level, and increases by 1d6 every two rogue levels thereafter. Should the rogue score a critical hit with a sneak attack, this extra damage is not multiplied. Ranged attacks can count as sneak attacks only if the target is within 30 feet.
The description of sneak attack does not say you can only get the extra damage once per round, it specifically says you get it anytime you meet the requirements. If you meet the requirements on 2 attacks in a round, both get the extra damage.

Okay, so. How does denying Dexterity bonus work in that situation? To put it in a scenario:
I'm a rogue, with an active Ring of Greater Invisibility. I manage to sneak up on a stationary enemy, but I'm not flanking. Two-weapon spec, I swing twice, and hit twice. Is the enemy denied their Dexterity bonus for both attacks?

The combat section specifically says that "Attacker is Invisible: +2 attack and defender loses DEX bonus". It's in the Combat Modifiers table. And as Jeraa showed, you can use Sneak Attack "any time" the rogue's target is denied DEX bonus.
QED.
So sneak attack with every attack you make. Always. Forever. And don't let any of MY players know how you got that (non-standard and grossly overpowered) ring...

Hrmmm....how you acquired a Ring of Greater Invisbility is a little suspect, such items simply shouldn't be available. However, when you are under the effects of Greater Invisbility, whether from a ring you shouldn't have to a a friendly spellcaster, then your enemies will be denied their dex unless they have some sort of sense or ability that denies that such as Improved Uncanny Dodge, Blindsight, Blind-Fight, probably several other ways too.
