I am confused as to how the spell "Suffocation" works from the advanced players guide. I get that if I cast it on someone on my turn that they need to make a save, if they make it they are staggered for their next turn. However, if someone makes the save initially do they need to continue making saves to avoid the effect for the next 3 rounds? Also if they initially fail the casting save, do they instantly fall unconscious on their next turn or do they get a save at the start of it for that not to happen for a round?

Nope. They save and no air is extracted from their lungs. Other than the staggered effect, they remain unharmed.

They make their save first. If they succeed, that round's effects are delayed until the next round. The spell doesn't say any other condition applies but it wouldn't be unreasonable for the GM to prevent speaking at that point until the spell duration expires.
This means that if they make just one save, they won't automatically die by the end of the spell but they may end up unconscious at 0 hp or dying at -1. An extended version of this spell would be quite nasty.

Can I get an opinion on Iridescent Spindle Ioun Stone which says "Sustains creature without air" and what happens if someone casts Suffocation on that person? I feel like the "Sustains creature without air" part implies that at worst the person may be staggered as the air is forced out of their lungs, but would otherwise be fine since they do not need air anymore.

This spell only affects living creatures that must breathe.
Someone using that ioun stone no longer qualifies as a creature that "must breathe".

I would rule that the ioun stone completely protects from the Suffocation spell, because they do not need air anymore. However, the caster should be entitled to a knowledge arcana or spellcraft roll to recognize what an Iridescent Spindle Ioun Stone does (since all of that same-looking stone does the same thing) and that the spell would be useless against that target.
