The question about costumes I felt didn't have enough options.
I mean, I'd go to a Ren Faire in costume or to an event at the right kind of con, but to a movie premiere? I'm nerdy but you have to draw the line somewhere.
Yeah, I thought the options for that question were really oddly specific in an unhelpful way. I picked the "movie premiere" even though I've never actually worn a costume to a movie premiere (but to Rocky Horror showings, sure, but I was negative one years old when that movie "premiered"). The first two options (no and just once) were definitely untrue, and likewise I don't dress up for EVERYTHING so that was untrue, so that was the closest I felt fit me.
Ren Faire should definitely have been a separate option, a lot of gamers and geeks I know costume up for Ren Faire (but for some of them, nothing else, not even Halloween).
Trying to honestly answer how many hours I play video games made me realized I have been indulging in waaaaaaaaay too much Harvest Moon and Roller Coaster Tycoon lately (I've been in a simulation mood lately). I think it's time to focus on my writing and music for a little while...
I'm curious that there were no questions about preferred video game genre. There's a big difference I think between say someone who exclusively plays shooters, someone who exclusively plays sims, and someone who dabbles in a bit of everything.
I'd also love to see a survey where you are given the opportunity to differentiate between causes of both helpful and unhelpful factors. The questions about stuff like health/weight and video games overlooks stuff and may seek to create a false correlation. I am overweight, but that is because of a) a sweet tooth b) a medication I must take that makes losing weight harder and c) lack of exercise in my earlier life (something I am correcting--maybe it's too late, but I figure any is better than none). Video games do not connect to any of the three.

Also, I breezed through the ratings 1 - 6 or whatever, before realizing 1 was best. I had to go back and change a bunch. If possible to make the note saying 1 is best larger, you might want to do that.
Same here, and wouldn't have caught it if I hadn't gone to the "review survey" section. Most online surveys or questionnaires I've seen use the exact opposite approach ( 1 is least, 6 is highest) so I suspect you're at risk of getting a lot of incorrect data if you don't change this or make it much more obvious that you're using the system you are.
