Neither. He has 3 attacks. Using a double weapon is the same as two-weapon fighting.

You should post the characters base attack bonus and feats. Its important.

Depends. Do you have the Improved Two Weapon Fighting feat? If yes, you get 4, if no, 3.

Nonsense. Using a double weapon does not give you the two-weapon fighting feat.

If your half-orc has a base attack bonus of +6, he gets two attacks. The two attacks can come from either end of the double ended weapon. If he has two-weapon fighting, he will get a third attack. If he has improved two-weapon fighting, he will get a fourth attack.

Nonsense! Two-weapon fighting isn't the same as the two-weapon fighting feat.

To clarify (in case there a new players reading this), the Half-Orc need not have TWF feat to make a third attack with his double weapon. All TWF does is reduce the penalties for Two-Weapon Fighting. Improved Two-Weapon Fighting does grant an additional attack.

It's also worth noting that even if the Half-Orc has all those feats he could still attack with both ends of his double axe without incurring TWFing penalties, so long as he only attacks twice (at +6/+1).

Here's a breakdown:

If your base attack is +5 or lower, you get one attack. Since the orc double axe is a double weapon, you could alternatively make 2 attacks as a full attack at -4/-8, attacking once with each end. If you have the Two-Weapon Fighting feat the penalty drops to -2/-2.

If your base attack is +6, you get two attacks with a full attack, at +6/+1. Again, you can choose to use the double weapon property on a full attack and make three attacks, at effectively +2/-2/-3. Again if you have Two Weapon Fighting the penalties aren't as bad: +4/+4/-1. If you also have Improved Two-Weapon Fighting you get an additional off hand attack so it's +4/+4/-1/-1.

None of these numbers include your strength bonus, any weapon bonuses, other feats, etc as we don't know any of that.
