RAW ("grapple" pg 199-200 Core) does not list a size restriction. So maybe a halfling "grappling" an Ancient Colossal Red Dragon is gamespeak for "the halfling's erratic yet frenetic movements work to enrage the great wyrm enough that it decides to try and stomp the pathetic flea for a bit [grappled] rather than release it's fiery breath of doom." (assuming the halfling's Grapple attempt succeeds, of course)

There are no limits. That's because in PF, grappling is different than in previous editions. In this one, it's more of a hampering than full body contact. If you grab the arm of something, you're grappling it. If you are on top of it on the ground, you're pinning it. 3.5's grappling was more akin to the pinning there is now.

That's mainly in specified for things like the Grab ability, which has size limits as opposed to grapple.
From the Bestiary:
Unless otherwise noted, grab can only be used against targets of a size equal to or smaller than the creature with this ability. If the creature can use grab on creatures of other sizes, it is noted in the creature’s Special Attacks line.

Ancient Red Dragon CMD 52. It's possible, I suppose... I don't think I'll ever see it happen though.
The online rule for Improved Savage Grapple says:
At 5th level, the brutal pugilist takes no penalties to Dexterity, attack rolls, and combat maneuver checks when she has the grappled condition. She also is treated as one size larger than her actual size when determining whether she can grapple or be grappled by another creature.
The rule in the actual Advanced Player's Guide book says:
At 5th level, the brutal pugilist takes no penalties to Dexterity, attack rolls, and combat maneuver checks when she has the grappled condition. She also is treated as one size larger than her actual size when determining whether she can be grappled using the grab feature or swallowed by another creature.

This question has been asked and answered before.
I envision it as the halfling leaping up, sticking his stubby little arms into the dragon's nostrils, grabbing fistfuls of nose hair, yanking down hard, and hooking his little feet under the dragon's chin so as to clamp its mouth shut and give him leverage from which to pull with.
