I seem to remember a magic item from somewhere that had tiny pixie-like creatures in cold iron cages on it. Some of the effects of incoming spells were redirected to the pixies. I'll do some searching to see if I can find it in my unindexed collection of stuff.

One of the players in one of the campaigns I run plays a warrior from a tribe of "barbarians" who takes the heads of his enemies, shrinks them and hangs them from his ceremonial spear.
When first presented with the idea I made him do the research on head-shrinking. He did and also took skill ranks in craft (taxidermy).
It has made for some interesting interactions in the more civilized areas.
To the topic at hand I would say something like a stirge maybe.

What is the tiniest monster that could, lets say, have its corpse on a necklace. Also, if you can pick something cool or interesting... +1 to you! Only used for roleplaying purposes btw.
Maybe not what you're looking for, but here's an idea. Not really for a PC, but maybe an NPC giant:
The large/huge/colossal NPC in question has some sort of immortal creature, such as an Imp or Quasit or something, trapped in a tiny glass ball just big enough to house the creature (not necessarily comfortably). The glass is either magically strengthened, or just strong enough that the tiny creature inside can't damage it. If necessary, the glass ball has a tiny Dimensional Lock on it to prevent the creature from teleporting away.
End result? A necklace or ring with a living immortal creature inside it. +10 points if the NPC has multiples of them (such as a ring on each finger or a necklace of a dozen orbs.
Edit: AMAZING IDEA! If the NPC in question is evil (which, let's face it- if he's eternally imprisoning immortal creatures, he's evil), his stronghold could be lit by countless glowing glass orbs set in sconces. Upon closer inspection, each one of these glowing orbs is actually a Lantern Archon imprisoned in a glass orb.
