250 is from the front. Those are eyeballs. 120 is from the back, hence the vertebral bones in the neck.

3-D has nothing to do with it. You cannot directly compare the brains, for instance, because in one case you are looking at the frontal lobe and in the other case you are looking at the occipital lobe and cerebellum. Different organs reside at different "heights". And many organs have a different appearance depending on what direction they are viewed from.

...which is, incidentally, exactly my point. My comment on 3D was to differentiate the images from something akin to TSA body scans, where the direction they are taken from is relevant. Your comment on the directionality of the appearance of various organs also presumes a 2D-image.

The lighter woman may be very muscular, fit, and skilled in martial arts. If that were the case, the 150 pounder could easily defeat the 250 pounder by striking her repeatedly in the head and not allowing herself to be grabbed.

The 250 lb. woman's picture, at the level of the head anyway, is a "more anterior coronal slice" in comparison to the 150 lb. woman's picture. It's a 3d image that's been chopped into slices, and the two patients have a different body habitus, and probably positioned a little differently.
