Fortitude save: 1d20 + 2 ⇒ (15) + 2 = 17
Spellcraft check to identify what just happened: 1d20 + 12 ⇒ (9) + 12 = 21, DC (15 + spell level, I think)
"Well, this is rather uncivil," Rhianna says, annoyed, looking up at the flying goblin. "If you choose to initiate hostilities, as there are not one but four clearly visible arcanists in our party, you are obligated by the Creed of the Nine to formally declare a challenge to the Duel Arcane! Humph!" The tirade is delivered as she stumbles away from the swelling fog (back up the road, away from the giant) and points her hand upward. "Veri yslou ra jeseza!"
Scorching ray at the goblin on the broom: touch attack 1d20 + 4 ⇒ (11) + 4 = 15, damage 4d6 + 2 ⇒ (1, 2, 3, 6) + 2 = 14.

I think Mahaffy won't get to fire until next round, since he must load his crossbow first.
Kai strikes the prone giant a fearsome blow (still waiting for that crit confirmation) but then is struck down himself by a mighty sweep of the club (for 21 points). The giant easily shrugs off Lyria's magic and clumsily struggles to his feet.
EDIT: Rhianna's ray hits some invisible forcefield, making it unbearably hot and scorching the goblin badly, but he does not fall.
Ka'kara is up, then the goblin and orcs.

Is there any additional effect from the cloud other than Roylenna (and Tika) losing their actions for the round?

It's a Stinking Cloud, so you'll be nauseated for d4+1 rounds. Apart from that and obscuring visibilty there is really no effect.

Erk. Looked at the spell description - d4+1 rounds _after_ leaving the cloud. Nausea: Unable to attack, cast spells, or do anything requiring attention. Only action possible is a single move action per turn.
Roylenna tries to escape the cloud by climbing down from the wagon seat, but her sudden movement makes her stomach roll. She's left hanging onto the wagon, feet just touching the ground, trying to stem the tide.

Ka'kara Furyheart

Out of reach, about 25 feet.
Actually, wouldn't be if I was enlarged. However, since that is not the case....
Ka'kara angered at the ambush, puts her spear butt into the dirt, reaches behind her, pulls out a javelin, and launches it into the air at the flying goblin.
I am assuming that being behind the wagon and now the stinking cloud Ka'kara cannot charge the giant or the orcs.
To Hit the goblin witch: 1d20 + 4 ⇒ (11) + 4 = 15
Damage: 1d6 + 6 ⇒ (3) + 6 = 9
She also steps 5' feet to the side to get a line on the orcs and giant.

Ka'kara throws a javelin at the flying goblin, but it bounces on the invisible barrier. Artival tries to fling a bolt but his eyes are still smarting from the cloud and he misses entirely.
The goblin speaks a magic word and vanishes from your sight, while the orcs advance, rushing up to Artival and Rhianna, ready to disrupt their casting.
Mahaffy, Rhianna and Lyria are up.

Rhianna Sanbury

Rhianna narrows her eyes, then reaches into her pack for a long, rolled-up piece of vellum, an air of barely-controlled energetic fury surrounding her. Then she looks down at the scroll, murmurs, "No, wait, that's not it." She pulls out a couple more, looks between them with a furrowed brow, then digs into the very bottom of her pack. "Aha! Here it is!" The rest of the scrolls get shoved haphazardly back into the pack, and she unfurls the last one she found and reads from it quickly. "Di ypeyo!"
Casting see invisibility from a scroll.

Lyria curses as Kai attacks the giant, and again after her spell fails. She moves out of the strange-smelling cloud while the battle rages around her and the wagon. On her way, she quickly assesses the party's situation and realizes that although everything seems chaotic, the only one hurt so far, pretty badly actually, is Kai. Swallowing her anger and pride, she advances behind him and quickly casts a simple healing spell on him.
Cast Cure Light Wounds on Kai:
1d8 + 2 ⇒ (5) + 2 = 7

Mahaffy

Mahaffiy thinks fast that the two bits are coughing their small lungs out but are in no other danger. He is confidant that Artival will deal with his opponent but hs is not sure that Rhianna has even noticed the two hundred pounds of screaming orc charging her ... He let fall his crossbow(free action), move behind the orc if it is possible to flank while drawing his axe and strikes the orc with Rhianna :
1d20 + 7 ⇒ (20) + 7 = 271d8 + 2d6 + 2 ⇒ (6) + (4, 5) + 2 = 17 or AC 25 and only 8 damage if he does not flank
Possible crit ! 1d20 + 7 ⇒ (5) + 7 = 122d8 + 4 ⇒ (7, 4) + 4 = 15
Damn , I wanted to see if Mahaffy could deal more damage than Ka'Kara ... Yes I know it's a futile hope

Rhianna Sanbury

Note that Rhianna has no weapon drawn and therefore does not threaten. And yes, she's completely oblivious to the orcs. ;)
However, I should point out that Rhianna took a full move action toward the rear of the group last turn, so even if the orcs can in fact get to her, they probably should be provoking a fair number of AoOs along the way...

Black Tom

Mahaffy runs over and cuts down one of the orcs, splattering Rhianna with blood but leaving her free to read her scroll. Lyria runs up dangerously close to the giant and heals Kai, who bats his eyes and awaken.
The hill giant takes a swing at Lyria, hitting her for 18 points of damage. Seeing her still standing, he grunts angrily and takes another swing, but this time Lyria's breastplate absorbs the impact.
Artival, Ka'kara and Kai are up (as our two favorite halflings clamber gasping out of the cloud).

I'm not sure of the positions of the orc and Artival, BT. If an orc is up in my face, I would like to 5' step back and cast color spray, if I can cast it and avoid hitting Mahaffy. If I can't avoid him, I will use one of my elemental burst rays. I am not going to post flavor until I know which of the actions I can execute. Let me know what is possible and I will make a post with attack and damage and save info. and all that. Thanks.

I'll give you the benefit of doubt here since I haven't provided you with a map. There is indeed an orc in your face, but you can step back and color spray it without worrying about Mahaffy.

Artival takes a step back to get aay from teh angry orc that apparently wants to kill and eat him. He decides to cast a spell at the creature, hopefully disabling it, and it could be questioned, if necessary.
Move action to 5' step. Standard to cast Color Spray. DC 13 Will save for the orc.

Ka'kara Furyheart

A look of disappointment crosses her face as the javelin bounces harmlessly off the witch. Seeing Kai and now Lyria in trouble she charges the giant letting out a primal scream as she does, attempting to impale it on her long-spear.
Raging log-spear powerattack and charge1d20 + 10 ⇒ (9) + 10 = 19

            Yes, thank you again, Roylenna.
Greta emerges from the cloud, carrying a large crate and cursing loudly. "Poor Frans! she exclaims. "He's got a sensitive nose, you know."
Artival envelops the surviving orc in a burst of swirling colors, but it manages to press on nevertheless. Ka'kara storms at the giant, but her spear is stuck in the filthy furs the giant is wearing.
The orc presses its attack, advances on Artival and hits him for 11 points of damage with a ferocious slash. The goblin continues zipping to and fro, turning visible as he targets Rhianna with his magical force. Magic missiles for 12 points of damage.
Mahaffy, Rhianna, Lyria and then the giant.

Rhianna Sanbury

Oh, you did not just throw a bunch of magic missiles at the evoker... Out of curiosity, how many missiles were there?
Rhianna stumbles back from the mystic assault, but after a moment, shakes her head and points it once more at the goblin. "Veri upeyo ra jeseza!" she shouts, and something like genuine anger is on her face as she does so.
Ranged touch attack for her second scorching ray: 1d20 + 4 ⇒ (2) + 4 = 6, damage 4d6 + 2 ⇒ (4, 3, 5, 1) + 2 = 15.
Ouch. Not a good time for that roll...

Ok, thanks.
Lyria raises her holy symbol and releases a wave of blue energy which washes around her, engulfing all who are near her - Kai, Ka'kara and the giant. As the wave touches them, some of their wounds close. Lyria herself, clearly feeling better, moves out of the giant's reach to the area between the giant and the wagon, still badly wounded but alive and able to heal. Trying to shout something out, it comes out in a much weaker voice than intended and no one hears her.
Channel Energy: 1d6 ⇒ 5. That puts Lyria on 5hp.

Rhianna sends a flaming streak at the goblin, but has difficulty hitting the moving target that he is, while Mahaffy runs to assist Artival, but the orc easily parries his blow. Lyria, who's still reeling from the blow, sends out a wave of refreshing energy, relieving herself and Kai somewhat.
The giant howls in fury and raises his club over his head for a fearsome blow with Power Attack and Cleave that almost knocks Ka'kara over (27 points of damage) before it descends on Kai for 25 points and smashes his skull, killing him instantly.
Artival and Ka'kara are up, then the orc and goblin.

Nice change !
BT , I'm not trying to contradict you . But I just want us to survive there ...
Yes, you only need to hit now. I think Ka'kara has taken advantage of that earlier if I don't misremember. Actually, since the giant has two attacks, it doesn't make a huge amount of difference. The benefit is that he can make both attacks at his full bonus, while he takes a -2 penalty to AC (also a change). And let me quote Pompey: "vivere non est necesse".

Ka'kara staggers from the powerful blow but recovers quickly and shoves her spear into the giants midsection. Using the backward momentum of the thrust she rolls free of the giant back towards the caravan.

The giant screams with pain and blood gushes from his side, but he still manages to catch the retreating Ka'kara with a fearsome blow (24 points), sending her somersaulting towards the wagon. The goblin screeches triumphantly while sending three more missiles thudding into poor Rhianna (for 7 points of damage). The orc takes a swing at Mahaffy but misses.
The bad news is that Lyria is now facing the giant alone. The good news is that he looks like he's on his last legs.

Ok. I don't see anything that Lyria has to finish the giant off herself, which puts her in quite a difficult position. That leaves the options of Summon Monster I or Channel Energy for 1d6 heal (cure light wounds is available but seems much less efficient now when the giant hits for 25ish and Lyria can heal for an average of 6. I prefer the Channel energy option but it depends on who's in the area I can reach.
BT, who's in the 30ft range from Lyria at the moment, and who will be there if she moves closer to the rest of the group?
Also, for healing purposes:
Artival is on 6hp
Rhianna is on 3hp (fixed)
Lyria is on 5hp
Ka'kara is at -1hp and needs to rage in order to keep standing (fixed)
Mahaffy is at max health and active
Tika and Roylenna are at max health and disabled.
Seems like Lyria will have a tough decision.

I thought the same thing, it seemed like she could move further back after leaving the staggered position. In any case, she's not in melee, but I don't know how far she is from the giant. Aside from that, assuming Ka'kara will rage, she'll be on 7hp, right?

Not in melee, but the closest living foe (which was what I meant). I guess her best bet is to back away twenty feet and channel, which would put almost everyone but the giant in the area of effect (let's say everyone for simplicity's sake).
