From what I have read, there are two different situations for the remaster. Rebuilding or major changes would not occur during a session. Changing feats, swapping out spells etc. would fall into this category.

Other changes, like the changes to the Spells that keep the same name but no have new damage, are treated like errata and applied immediately.

I think this post and the one that follows apply here, but it is not 100% clear.
