Maybe useful as a thread for other issues, but a big one I've just seen is the incoherent Kintargo map on page 172 of the main book or page 12 of the interactive maps pack.
The map has a bunch of numbered pins (e.g. G8, J5), but there's no legend to match those pins with location names.
The initial letter almost certainly matches the district name, but without a legend it's just a bunch of numbers.

EDIT: The previous version of this post claimed they'd just reprinted the Hell's Rebels map's labeling. I just took a closer look at the numbers and that's wrong.
The way they seem to have done it is that the order of the locations in the text correspond to the number on the map. So O6 is the 6th location mentioned in the text for Old Kintargo (which is the Old Harbor). I'm wondering if they had the numbers in the text originally but took them out.
What I find funny is that they already reprinted this map in Curtain Call at least twice. So this is one of the most reprinted maps in 2e and they do it a different way each time.

Mythkeeper has noted that two locations are written in the text of the book as being on the border between Cheliax and Corentyn, but the map shows them both deep within Cheliax territory:
New Anglemire* (built on the ruins of Anglemire),
and Upper Macini (bonus error: incorrectly written on the Corentyn map as Mancini).
The section about (Lower) Macini in the Cheliax chapter also seems unaware that half of the city is now in Corentyn, unlike the Corentyn chapter's piece about the same city.
*at least definitely on the Corentyn version of the map. The Cheliax version is more zoomed out so it's harder to tell, but still looks to be within Cheliax borders.
