Does the enemy take mental damage immediately or only at the end fo their turns now?

Depends on what you choose, but it's either direct or persistent, not both simultaneously.

The feat allows you to choose to deal persistent mental instead of the normal mental. But you must make said choice before the enemy makes his own choice.
