The baseline is equal to XP, so 4 for a scenario. 1 for a Bounty or old Quest, 2 for a new Quest or some short sanctioned adventures like the ones in Dark Archive, 4 for sanctioned one-shots, 12 for full-length adventures and AP books.
There are multipliers. x2 for GMing, commonly x1.25 for events, but there are higher ones. These stack.

No, You can get credit for a full length adventure or part of an adventure path* by playing one with your local group or even potentially a home game as long as the GM submits the forms via OP (This does require the GM to correspond with the local Venture Officer, so I'm not sure of the specifics). Playing an adventure all in one sitting is typically only possible for an entire group of adults if they've taken off time for a convention, though.
I believe the qualifications for getting the multipliers has been lowered this year as well, although not being a Venture Officer myself I'm not privvy to the specifics.
*I believe you typically get 1 chronicle/book. So playing through the entirety of Abomination Vaults, which was originally a 3 book Adventure Path before being compiled, would give you 36 Achievement Points. Age of Ashes is 6 books, so you'd get 72 achievement points once you completed the adventure path.

The GM just needs to create an event via the website, select the adventure/AP volume as the game being played, and then just report as normal afterwards. No VO need be involved.
