Does wearing it actually make your skin look any different? Or would it look normal but feel as tough as,say, rhino skin to the touch?

It's barkskin as an item. The spell doesn't say, but I've assumed the effect is visible and bark-like.

I would say no. It's such a standard piece of NPC gear that I'd expect it to be mentioned if everyone above a certain level looked like a walking tree.

Spells and spell-like items aren't identical.

Barkskin never mentions it makes your skin look bark-like either. I think it’s just a catchy name. It’s like how bull’s strength doesn’t make you look like a bull.

The spell itself just says 'toughens the skin', without a mention of direct appearance changes.

I agree- it probably refers to the fact that it makes your skin harder to cut into, without having the extreme hardness of stoneskin. Thus, chopping your arms off feels practically like chopping into a tree trunk.

Somehow I never noticed barkskin doesn't say it makes your skin look like bark. That said, I imagine it would (even if the amulet doesn't) since it's transmutation, whereas, say, stoneskin is abjuration so it probably doesn't really turn your skin into stone but instead makes some kind of (invisible) force field that absorbs damage.

I kind of imagine that the same spell can have varying visual results depending on the type of caster, casting tradition/school, and just personal preferences.

I would imagine that druids would have a far more plant like version with bark, for example.
