I'm curious, I can't find anything specifying. Do my grapple bonuses apply to my CMD when someone else is trying to break my grapples? Like, I've got a grappler with about 12 points to her CMB specifically for grapples. Does all that stuff stack against someone trying to break grapples I've established? And when I'm tying someone up?

Generally, no. Only if the ability states it does (such as the Improved Grapple feat).
That is one problem about being grapple focused. Luckily some racial favored class bonuses offer +1 CMD vs Grapple.
If you want to ensure that someone doesn't break out, investing your points there will be worth it.

Limbo is correct, CMD is only raised if the ability/feat/whatever states that it actually raises it. Most abilities only raise the CMB to grapple, not the CMD.
I do not know what kind of class you are, nor your race, but various classes/races can get bonuses to CMD (straight CMD or only versus grapples). It is also possible to throw money at this problem: the Frosted property for armor/shields gives a +4 on CMD versus grapple, the Amorphous property gives a +5 competence bonus on CMD versus grapple, and the Pauldrons of the Serpent give a +2 versus various Combat Maneuvers (this one does take up the shoulder slot though). Also, anything that raises your base CMD can also be used (Strength/Dexterity Belts, etc).
As for feats, there is Improved/Greater Grapple and Unfair Grip (gives the opponent a penalty on escaping. The other feats give bonuses to the CMB to grapple, and not to CMD (unless I missed one or two, there are so many feats currently, can't remember them all).
You could also rely on spell support: Resinous Skin gives you a bonus on grapple, and the opponent a penalty to escape. Freedom of Movement on yourself forces the opponent to use Escape Artist to escape, as a counter-grapple automatically fails. There is even low level support, as you can Grease yourself and gain a +10 bonus on your CMD versus grapples (but for some reason no penalty to establish/hold them yourself).

Also remember that any dodge or deflection or other special types apply to your CMD too. It seems to be a rule that many aren't aware of.
Well I'll be! I was in that category of people who didn't realize that handy piece of info! Just went and double-checked the PRD and it is in there clear as day. Monsters had enough issues escaping my monk's grapple before, but they should have never been able to escape!

Yeah, monk's CMD is nuts, 10+str+dex+wis+level+deflection+dodge means very big number can be reached.
