What potion gives you the mutant template?

I once found a potion that gives 1% chance to acquire the mutant template but can't find it anymore,help please.

Yup, but that would be a chance of 0.1%.

It's Mutagen. Green glowy stuff. Mostly kept in big yellow barrels in the subways under New York City. Keep it away from turtles and rats.

I think Waters of Lamashtu also cause mutation, but it's less mechanical and more if the GM feels like it.
