Right now by RAW, you only encounter a cohort when you would banish it from a location deck if you would try to close a location that contains a villain in it. In all other cases (including when closing a location that does not contain a villain), the cohort is simply banished (and therefore removed from the game).
If you succeed at meeting the When Closing requirement, search the location deck for villains. If you find any, banish all nonvillain cards from the location deck. The location is not closed—but at least you know where the villains are! If you would banish any cohorts this way, you may encounter them instead.
If you didn’t find any villains, perform the When Permanently Closed effect: First, apply any effects that say “before closing.” Then banish all of the cards from the location deck; it is now closed.
The above rule quote is the only mention of encountering cohorts from location decks instead of banishing them. The later rule for when you don't find villains does not say to encounter cohorts instead of banishing them. Furthermore, if you would need to banish cards from location decks for reasons other than closing them, that would also kill off cohorts.
I suggest that we instead remove the rule in the closing section on page 15 and put the rule on page 23 under the Cohorts section (blue text indicates the suggested addition).
If you encounter a cohort in a location deck, you automatically acquire it. If you would banish a cohort from a location deck, you may encounter it instead. Otherwise, if you would banish a cohort, remove it from the game instead; it may not be used in future scenarios, even if it’s listed on your character card.

Corrected verbiage is in the online version of the rulebook, and follows the adjustment made via the Wrath of the Righteous FAQ.
I'd prefer to retain the rule in the Closing a Location section, but adding it to the rules for cohorts would be a nice addition (providing a better chance that players would notice/remember the rule).

I AM quoting the PDF rulebook; the change does not sufficiently address the problem, as stated above.
Putting it on the closing section requires stating it twice (once for finding a villain in the location, and again for not finding any villains), and it still doesn't cover banishing a cohort from a location deck for reasons other then closing it. Putting it in the cohort section as a general rule covers all cases of it being banished from a location. If someone doesn't read that section, then they wouldn't know to remove cohorts from the game instead of banishing them either, so no issues there.
