What would happen if I spread open a portable hole on the back of a Large creature I was riding? Like, would the creature be harmed in some way? Possibly killed by a sudden hole appearing through it?

The creature would not be harmed. The Portable Hole doesn't punch holes in things, it opens up into an extradimensional space.
