I had a few deaths across the campaign. Two stood out as most amusing and satisfying.
1. The Hunting Horror in book 5. The party had encountered it twice before and ended up running away. Each time they used the same tactics: one spellcaster would cast Haste, another would buff the Paladin with Freedom of Movement, the Paladin would activate his Fly spell and charge. The HH, being reasonably intelligent, guessed they'd do the same again. So it used its Limited Wish ability to cast Antimagic Field on itself. (We homebrew spells like this so the radius extends 10' beyond the creature, rather than the creature having a 10' Antimagic Field within itself, because that would be stupid.) The Paladin, charged in and just as he reached the HH lost all his magic, including pretty much every Paladin feature, magic items, buff spells etc. The fight was now between a non-magical human with a mundane greatsword and a non-magical gargantuan beast with a Str of 34. RIP Paladin.
2. Captain Armel in book 6. Again the Paladin. Armel saw the party and didn't fancy taking them all on at once, so he challenged the Paladin to single combat. The Paladin, being a Paladin, stupidly accepted. The first blow of Armel's vorpal greataxe was a critical hit. Crucially, the rest of the party was more than 30' away so couldn't use their many abilities that forced re-rolls. RIP Paladin.
