that might seem like a stupid question but the german rules are not very clearly there. I found an entry in the english rules which was far more informativ. it says that the secondary effect applys the round after the initial effect while the initial effect (like the word says only applys once no matter how often someone is poisoned)
so if i get this right: i hurt someone with a sleeping poison. if she fails her save, she gets one CON DMG and the round thereafter if she fails again she falls to sleep?

Poisons have an initial and a secondary effect. Typically they are the same, but a number of them are different. Initial effects happen instantly upon contracting the poison. Secondary effects happen afterwards. When the secondary effect occurs depends on what the onset period of the poison is, but yes the majority of secondary effects begin the round after exposure.

If it has an onset time the initial effect doesn't happen until the onset time is over.
It goes like this - Poison: save - onset time - nothing happens.
Onset over - save - if fail (2nd time) initial effect. - secondary effect then happens subsequently.
