Rules for creating two-headed races

Are there any rules for creating a player character race with multiple heads? Monstrous traits list multi-armed and quadruped but I haven't been able to find anything for multiple heads other than this:

Sadly there's no RP conversion for race balance. Any help on this would be appreciated.

Nothing official, unfortunately.

I have a PC in one of my campaigns who's dealing with this (mutations!), so it's of some interest to me as well.

You could just take some traits to simulate a two headed race. Such as..

Dual-Minded (1 RP)
Stubborn (2 RP)
All-Around Vision (4 RP)

Is there some benefit in particular you want the race to gain from having two heads? Maybe we can determine a good RP cost for it.

Specifically looking for this, which is effectively an improved version of the two-weapon fighting feat:

It's not balanced.. two headed races can be effectively two classes, such as the infamous Ettin Monk which had two sets of flurries.

This is on the level best reserved for monsters.

I made an anthropomorphic cerberus race. Their multiple heads work like a hive mind. They gained a Perception bonus, a bonus against illusions, and the ability to sleep with one head awake.

There's not much precedent for multi-headed creatures gaining tons of abilities. It just gives them a Perception bonus. Even hydras don't get all-around-vision.

We could try and break it down into it's component parts.

The easy ones:
2 static Bonus Feats: doesn't get any easier(4RP)
Extra Bite: closest equivalent is either 1RP natural attack or 2RP for 2 claws - it only adds an extra attack so stick with +1 for now (1RP)

The not so easy ones:
Improved Multi Attack
a) no penalties: Most often offsets -2 to hit, but can be abused with one-handed weapons in off-hand to offset much larger penalties. Also I don't think it offsets the -5 to hit for secondary attacks as that is not a penalty for multiple attacks. However I believe that primary attacks will not be demoted to secondary attacks when combined with weapon attacks. See in Darkness totally negates all penalties associated with darkness that are similar in scale, but is more circumstantial - lets ad 50% (6RP)
b) 2 attacks as standards: An extra attack, but with action economy and a damage bonus - lets take claws and double it (4RP)
2 heads are better
a) swift action SA: close to an extra attack, but action economy that conflicts with other swift actions (not much of a penalty), lets do the same as 2 attacks as a standard. (4RP)
b) +2 DC/+2 to hit: Ability Focus or weapon focus+greater weapon focus, still essentially only equal to a feat (2RP)

First draft puts it at about 21RP (15 without '2 heads are better')

That is obviously just a quick and nasty first attempt!


There's one humanoid race that has two heads I can think of off the top of my head

Minor Emerald Spire spoiler:
In the module on one of the floors there's a modified serpent folk sorcerer with two heads. I can't remember what it did to the CR.


You're desired Improved Multiple Attacks is just completely overpowered.

It is not suitable for players.
