Ok, I think I'm a bit confused at how defensive casting works. Let me see if I have this right:
Mage threatened by Fighter: Must cast defensively or lose the spell.
Mage threatened by Fighter w/Spellbreaker: Must cast defensively or lose the spell & possibly get hit by attack of opportunity.
Is that correct?

Yes, except for the must part. A spellcaster doesn't have to cast defensively if he doesn't want to (he just gets an AOO against him).

Combat casting feat.
If a wizard is webbed is this a melee attack? If he can cast aspell does he have to make a concentration check and does Combat casting feat apply?
Technically he's just entangled, which forces a mandatory concentration check (15+spell level) - it's not an attack. There's a whole list of situational concentration checks if the spell has somatic components (like being grappled, underwater, in a hurricane)
Combat casting would not help here, as it only helps in the defensive casting concentration check- concentrating to avoid provoking attacks of opportunity. He doesn't have to cast defensively, but if he gets hit he has to make ANOTHER concentration check based off damage not to lose the spell.
