Can a witch use a hex while grappled? If so is there a concentration check?

Yes, and only if it is an (SP) ability. You never need concentration checks for (EX) or (SU) abilities.

Yes. We have a local witch famous for being grappled by a Black Tentacles spell and just relaxing, going limp, and spending the entire battle hexing away without impediment. He basically won the fight by himself, and never even attempted to leave the tentacles. :P

Huh. That witch must have had pretty good hit points to shrug off 1d6+4 damage per round. I mean, that's not a HUGE amount, but then witches tend to the squishy end of the spectrum.

Pretty much the only way to stop a Witch from hexing is to make them unconscious (though blinding them will eliminate quite a few hexes as well).

Most hexes are Supernatural abilities and don't require you to be able to move, so being pinned is no problem.
Hexes that function as Spell-Like abilities can still be used, but become more difficult. Pinned is a worse form of Grappled, so to use a Spell-Like ability you need to make a concentration check for trying to use spells while grappled.

How do I know if Evil Eye, Cackle, Misfortune and Slumber are spell-like abilities? Where do I find that? Would these be usable while pinned?

All of those are supernatural abilities, not spell-like abilities.
Hex descriptions, like most class abilities, always* have (Su), (Sp), or (Ex) after them. (Sp) indicates a spell-like ability. Check here for the basic list of hexes from the witch class description.
*At least, I know of no counterexample.
