There are many companion and familiar PDF sheets on the web. Some of them small(2-3 pages) and others huge(4-6 pages). But not many of them are fillable and small at the same time.
Here is a custom made fillable Animal / Companion / Familiar PDF Sheet modified for the Pathfinder rules set. It consists of only one page so it is really easy to manage and update the pets either by your DM and/or the PC's. I have included the original sheet I found on the web and the modified version if someone desires to get them both and see the differences.
The original design was made by Ronny Hart for the 3.5 rules set (of D&D) and all the modifications for the Pathfinder rules set were done by me. It only has the stuff from the Druid class of the Core Rulebook so if you feel that something is missing please don't hesitate to tell me.

+ Added a script to get the current date automatically. If someone wants to write their own date it's fine. The script only works if the Date box is blank.
+ Moved and resized some of the previous boxes slightly to fit the actual box they are in.
- Removed some unused skills and skills modifiers.
+ Added the Pathfinder Animal skills. (Found on the Druid class inside the Core Rulebook)
- Removed the "-5" modifiers that were showing if the stat blocks were blank. It still checks for the modifiers but not if they are blank. (For aesthetic reasons nothing more.)
+ Moved the Special Abilities and Notes higher up to compensate for the empty space caused from removing the unused skills.
For anything else about the sheet please don't hesitate to comment.

Because I cannot edit my previous threads/posts, I posted the fixed links below. For any new versions that might pop up, I will post them here below so be sure to always search for the latest post I made on this thread.
Am I supposed to be able to fill the sheet in on screen? If so I cannot. Also the spaces for the skill bonuses have 0's in them so even if I printed it out I still couldn't post the skill modifiers.
Otherwise a neat sheet.
You have to download the actual sheet and then fill the spaces. The 0's on skill bonuses will be updated automatically as soon as you put the STATS on.
I see you neglected to add CMB and CMD to the sheet.
BTW after I downloaded it and opened it up it does auto fill.

A new version of the file is uploaded. CMB and CMD is added with the addition of some javascript to remove those numbers on the form.
Go and check it out.
If you find any bugs please let me know. I will fix them a.s.a.p!

+ Added Base Attack Bonus fill able field.
+ Added CMB and CMD fill able fields.
+ Added javascript code to remove the numbers on the form for a printable version on the go.
+ Added a non print-able Clear button to clear the form in one click.
~ Cleaned up all the form fields. (Overlapping issues and other aesthetic stuff.)
~ Changed the order of calculations to include the CMB and CMD change.
Could you make one made to be printed blank? I really don't want to go into the PNG and edit out ALL the grey text in the fields to be above/below it.
Thanks for the suggestion . I will definitely do that. It is going to be a different pdf file with Blank in it's name.

I made this Animal Companion / Familiar sheet for Pathfinder from the Paizo character sheet!
Open in Adobe Acrobat and create fields to make it fillable. Also love using Drawboard PDF for my character sheets.
