You have to download the actual sheet and then fill the spaces. The 0's on skill bonuses will be updated automatically as soon as you put the STATS on.

There are many companion and familiar PDF sheets on the web. Some of them small(2-3 pages) and others huge(4-6 pages). But not many of them are fillable and small at the same time.
Here is a custom made fillable Animal / Companion / Familiar PDF Sheet modified for the Pathfinder rules set. It consists of only one page so it is really easy to manage and update the pets either by your DM and/or the PC's. I have included the original sheet I found on the web and the modified version if someone desires to get them both and see the differences.
The original design was made by Ronny Hart for the 3.5 rules set (of D&D) and all the modifications for the Pathfinder rules set were done by me. It only has the stuff from the Druid class of the Core Rulebook so if you feel that something is missing please don't hesitate to tell me.

+ Added a script to get the current date automatically. If someone wants to write their own date it's fine. The script only works if the Date box is blank.
+ Moved and resized some of the previous boxes slightly to fit the actual box they are in.
- Removed some unused skills and skills modifiers.
+ Added the Pathfinder Animal skills. (Found on the Druid class inside the Core Rulebook)
- Removed the "-5" modifiers that were showing if the stat blocks were blank. It still checks for the modifiers but not if they are blank. (For aesthetic reasons nothing more.)
+ Moved the Special Abilities and Notes higher up to compensate for the empty space caused from removing the unused skills.
