I've read that a heart dripping blood was the symbol for a drawn out and tortuous death. A spear or dart the symbol for a violent death. An hour glass meaning that time is running out for the victim to surrender peacefully. The black flag was used as a quarantine symbol for disease aboard ship and it was also associated with funerals and mourning. A red flag was used as a symbol in naval warfare that no quarter would be given.
I presume that the Skull & Shackles symbol represents death or slavery to the victims of the Free Captains. I am also calling the ensign of the Shackles "The Skull & Shackles." I presume that the black on this flag represents death and mourning and the red meaning that no quarter would be given.

Putting it all together: "This is your last chance to surrender peacefully, before we subject you to a violent, torturous death."
Appropriately enough, the crew who chose that flag also named their ship the Last Chance.
