The Witch used a Baleful Polymorph to turn it into a Rabbit. It rolled a Natural 20 on the Will save. At the time I ruled it now had Animal Intelligence, although I suspect I was wrong.
The Blodeuwedd Druid is now looking after it and I have various questions to answer.
1) Should it have remained Mindless (if I got it wrong I won't change it now, but would like to know what was the correct ruling for future)
2) How can Baleful Polymorph be reversed? It doesn't have a natural shapechange subtype and the spell says other polymorph spells/effects won't work. It's Duration is Permanent so that means it can be Dispelled yes?
3) If it does get transformed back is there any RAW way of maintaining it's INT? I can just make a GM ruling but would like to know the actual rules before I decide to modify them. I was thinking if they put a Headband of Int on it and converted it back it might retain the Headbands Int score.

Saving Throw: Fortitude negates, Will partial, see text; Spell Resistance: yes
If the spell succeeds, the subject must also make a Will save. If this second save fails, the creature loses its extraordinary, supernatural, and spell-like abilities, loses its ability to cast spells (if it had the ability), and gains the alignment, special abilities, and Intelligence, Wisdom, and Charisma scores of its new form in place of its own. It still retains its class and level (or HD), as well as all benefits deriving therefrom (such as base attack bonus, base save bonuses, and hit points). It retains any class features (other than spellcasting) that aren't extraordinary, supernatural, or spell-like abilities.
Any polymorph effects on the target are automatically dispelled when a target fails to resist the effects of baleful polymorph, and as long as baleful polymorph remains in effect, the target cannot use other polymorph spells or effects to assume a new form. Incorporeal or gaseous creatures are immune to baleful polymorph, and a creature with the shapechanger subtype can revert to its natural form as a standard action.
