Here's the Cap of Human Guise; note that it does not actually cast disguise self, but rather acts like disguise self.
My reading, and I may be wrong about this, is that it allows the wearer to appear as any of the given forms.
So, I've got a character about to grab a level in Living Monolith, so he's going to be going around Enlarged half the time. What I'd *like* is for him to look like a small kid whipping a Large Adamantine greatsword back and forth. Because it would be awesome, but for little reason more than that.
Can this be supported, RAW?

Seems to work just fine. It wouldn't change the items he's holding, so yeah, large greatsword swung by a human child. Nicely done.

A bit odd, I know, that the "Cloak of Human Guise" and the "Cap of Human Guise" would be pretty significantly different in effect.

The size difference would eat at your disguise bonus.
You get the look, but not many will believe it.
The spell description from Disguise Self says that you get a +10 to the disguise check, but the skill description for disguise says that you would take a -10 for each size category difference from your own. So I figure it would amount to +10 for the hat, -20 for the two size categories of difference -2 for a different race = -12 to disguise yourself as a child.
Difficult, but not impossible.The tricky part is imagining what a Living Monolith disguised as a 4 foot kid would actually look like. Perhaps you could just paint a picture of a child on your leg, and the rest of your body could be painted the colour of the sky.

So, one thing I'm not clear on. The disguise self spell says "If you use this spell to create a disguise, you get a +10 bonus on the Disguise check."
Is there an alternative to using the spell to create a disguise? I'm just wondering about the presence of the "If"
Good question. It's a bit of a tautology, isn't it? I have absolutely no idea what else you could use it for.
