Description
This belt is a thick leather affair, often decorated with huge metal buckles. The belt grants the wearer an enhancement bonus to Strength of +2, +4, or +6. Treat this as a temporary ability bonus for the first 24 hours the belt is worn.
so after 24 hours of wear time, does the belt always grant the wearer its bonus? or does the bonus fade from the item after 24 hours and it no longer gives a +2 Str?

For the first 24 hours, it counts as a temporary bonus for things that are/are not granted by temporary bonuses. For example, if you put on a Periapt of Wisdom +2, it doesn't actually grant additional spell slots for a Wisdom-based caster for the first 24 hours (because temporary bonuses don't give additional spell slots), though it would still grant bonuses to Will saves, Wisdom-based skill checks, and Wisdom-based spell DCs. After 24 hours, it becomes a permanent bonus, and thus grants every benefit that an actual score of 2 higher would grant.
The bonus still goes away, of course, when you take off the item. Even though it's called a "permanent" bonus, it's still tied to the item.

Officially someone with a 3.X like cloak of charisma never remove it if he want to fully benefit from it, and he never remove his headband of wast intelligence, independently if it has the shape of a thin circlet or a heavy, ornate crown that will strain your neck after wearing it for half an hour.
They must sleep "very well", even without speaking of other activities.
My fluff is that you can remove that kind of items for a limited time (30 minutes or so) an still fully access they abilities when you don them again.
Not using this kind of home rules mean that a guys successfully casting dispel magic on a wizard headband of wast intelligence will immediately remove the spells grated to him by the intelligence increase, with the usual problems of determining what spells were granted (you can substitute the other mental stat enhancing items for the other classes).

so long as you wear no other item in the slot, and nobody else wears the item, I think the grace period is a fine idea... That preserves balance without creating weird problems and characters who can't take off the belt/girdle that is at least sometimes (presumably) worn *over* the armor that they have to remove to sleep... Yea.
