As far as Anti-magic goes, I have an area in my campaign world where goblins discovered some menhir that send up a radius of anti-magic. Being fearful of magicians and an over all cowardly lot, they set these up [stealing them from where they originally where] in and around their lands. Go into the goblin forest you can't cast spells or use magic items. So really it depends on the game world.
Oh as an aside the PCs took one of these from the forest and placed it in the middle of the main city. Dragons don't seem to like them.

I think this should be filed under "something you never need to worry about because no Good DM will ever make this a problem."
Disjunction is a spell the PCs should never be hit with, ever unless they're already naked and just using spells. Antimagic is something that can be readily bypassed, if by more clever means.

And all enemies should use pillows instead of weapons and wear armor made of cardboard...
There is no reason a good GM should not try and make the world challenging for his players and no reason players should be surprised when their obvious weaknesses get slapped.
