Do you threaten all squares within reach in 3-dimensional space? So does a normal medium PC threaten the five-foot space directly above him, so that a flying creature who passed directly over him would provoke an attack of opportunity? What about the five-foot space above-and-beside him (on a diagonal)? Would a flying creature who lands in front of a medium PC provoke an attack of opportunity for moving through that space?

Along the same lines, do falling creatures still provoke AoOs in this way?

do not forget to think 3d when using a reach weapon against a flyer in a place with a ceiling


And it gets tricker to visualize when you're dealing with Large or Huge creatures, with 10' or 15' reach. Flying 20' above the floor is perfect safe against medium creatures, even with reach weapons. Not so much vs Large creatures with natural reach. On occasion we've had to draw a "side view" on the battlemap to get a good grasp of it.
