Just wanted to clarify a couple of things here...
1. When a target is Grappled they take a -4 to their Dex, and a -2 on all attacks and maneuvers except those made in the grapple itself. The one who initiated the grapple also gets +5 to maintain. Hold on though... Dexterity applies to your CMD. So does that mean the maintain is effectively at a +7 due to the Dex penalty? I think we've been forgetting that little bit at our table.
2. Does a Druid using Wild Shape or any other caster using Beast Shape II to take on a form with the Grab ability gain the +4 bonus to grapple that comes with the ability?
2a. If so, does that bonus stack with the bonus granted by Improved Grapple and/or Greater Grapple?
3. Greater Grapple allows you to maintain as a move action instead of a standard action, thus allowing you to move/harm/pin the hapless opponent twice in a round. What if you have the Rake ability however? Does the Rake only happen once per round regardless, or once per maintaining the Grapple?

3) Ask your DM. My interpretation is if you begin your round grappling, you can take your rake attacks listed once per round if you maintain the grapple.
Agreed on all of these answers, although I'd add that Rake only mentions getting two free attacks if you begin the round grappling; it doesn't say anything about maintaining the grapple.

Here's a question: Is using an Escape Artist check to break a grapple considered a grapple check? (Answer determines effective modifier of attempts to break on round 2+)

Round 1, Initial grapple check: No modifiers (Your CMB vs his CMD)
Round 1, first attempt to break:
a) Using grapple: Your Dex penalty gives him an effective +2 to break (his CMB vs your penalized CMD)
b) Using Escape Artist: Dex penalty applies to both, no effective modifier. (His penalized Escape Artist skill vs your penalized CMD)
Rounds 2+, attempt to maintain: effective +7 to maintain (your +5 kicks in, his CMD suffers from the Dex penalty)
Rounds 2+, second attempt to break:
a) Using grapple: Effective -3 to break (his CMB vs your penalized CMD +5 circumstance bonus)
Now, if the Escape Artist check is NOT a grapple check:
b) Using Escape Artist: Effective no modifier (His penalized Escape Artist skill vs your penalized CMD)
But if the Escape Artist check IS a grapple check:
b) Using Escape Artist (G): Effective -5 to break (His penalized Escape Artist skill vs your penalized CMD, +5 circumstance)

No, there's no +5 bonus to your CMD; it's a +5 bonus on YOUR grapple checks, and HE'S the one making the check in this case.
So it's an effective +2 bonus, as 1(a) above.
Now, if the Escape Artist check is NOT a grapple check:
b) Using Escape Artist: Effective no modifier (His penalized Escape Artist skill vs your penalized CMD)
Yes, this is correct, same as 1(b) above.

If Player A is trying to escape from Player B, then Player A is making a grapple check and Player B isn't.
