Well, fangs are essential. Prefferably dripping with something dark and viscous.

Tentacles? You can't go wrong with tentacles. Maybe some extra appendages for good measure. Oh! And an insect-like head.

Giant aquatic thing with a vaguely humanoid torso, a gaping shark-like maw at the top between it's shoulders where a human would have a neck and head, and at the end of it's arms, it has headless krakens for hands.

It's lower body is that of a merfolk, only longer than a sea serpent, and trailing back into the sea, and pushes itself forward with a combination of tail motion and 'walking' on thick fins along the sides of it's tail and torso, similar to the movement of a walking catfish, while it's twenty-odd tentacles and arms snatch up fleeing fisherfolk to dangle into it's pit-like central maw.

Lots of AC, lots of hit points, lots of DR, and, due to it's colony-creature nature, the most powerful save or die spells, if they affect it, only affect a part of it, equal to say, 20% of the whole, and costing it certain abilities (blowing off one of it's kraken-hands, for instance), based on which part is targetted (roll randomly if the caster doesn't specify which part is is attempting to petrify, polymorph, disintegrate, etc.).

The problem is that any time I try to stat something "epic", I quickly begin to shake my head and come to conclusion that 3.5 epic rules are a clunky mess. Granted, I did re-stat Karzoug as an epic wizard, need to dig out that and post someday :)

A tarrasque with 20 levels of sorcerer is CR 35.

(Just don't forget to apply the elite stat array, preferably increasing its Intelligence and Charisma.)

Edit: Or, if you want to play hardball, make a tarrasque with 20 levels of cleric.

Edit: Essentially, take any high-end non-epic monster, see what its stats would look like if you added 20 class levels, and base your monster with a CR in the 30's off of that.
