The description seems pretty clear that they three work together to use one each round. As to the initiative question, if you've rolled three separate initiatives for each hag, seems the best way to go about it would be to have the other two delay down to the lowest initiative roll and have them act in concert at that point in the order. Alternatively, since they may want to act in concert, you could always just roll once and apply the lowest initiative score.

Correct; it takes all three hags to use coven magic. The Hags would probably need to all go on the same initiative rank, but if they don't then the magic activates on the third hag's action in the round.
